As part of a multidisciplinary care team, the Outpatient Dietitian (RDN) provides nutrition education and care to the patient. The primary responsibility of the Outpatient Dietitian is a consulting role providing patients (individually or group classes) education including evaluation, documentation, and the development, implementation and coordination of the outpatient nutrition education program to optimize patient outcomes and health. In addition, as a Certified Diabetic Educator, the Outpatient Dietitian will provide diabetes self-management education and training to individuals and groups in an outpatient setting according to the scope of practice, standards of practice and professional performance for Diabetes Educators. This position is assigned to specific cost centers, programs or projects to provide nutrition services compatible with established standards of practices and in compliance with state and federal regulations. This position coordinates with management and appropriate staff to promote a positive image of HUMC as an expert leader in the area of Nutrition.
