The RPM Telehealth Registered Nurse (RN) role is an in person role located in Clearwater Florida. There is a possibility of a hybrid model after 1 year. The position offers an exciting opportunity to work using modern technology to provide telehealth nursingcare to patients at home to improve health outcomes in chronic illnesses. The Telehealth RN will service a panel of patients andprovide virtual health and wellness coaching/education based on monitoring and analysis of health vital signs. The role will work under the supervision ofPhysicians andNurse Practitioners who are dedicated to enhancing patient care using telehealth and remote patient monitoring.
JOB FUNCTIONS - Meet with patients who are in the RN patient panel via audio or video calls regularly to discuss vital signs and chronic condition management and education.
- Monitor and trend patient vital signs and clinical alerts
- triage, review, respond, and document as appropriate
- Documents interventions / individualized plan of care
- Educate patients on the significance of vital signs and abnormalities
- Utilize a standardized treatment approach to provide high level of care to patients
- Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects ofcare delivery
- Serves as advocate for patients and families in response to their needs
Qualifications Education - Bachelor'sDegreein Nursing Required - Registered Nurse
Work Experience - Minimum, 2 years of experience managing chronic medical conditionsincluding Hypertension, COPD, Heart Failure, and familiarity with medication used forthese conditions
- Prior Remote Patient Monitoring Experience is a plus
- Case management experience managing a panel of patients is strongly encouraged
- Experience with treating patients via telehealth is a plus
Knowledge, Skills, and Abilities - Management of a panel of patients
- Prioritization and time management between multiple activities
- Excellent written and verbal communication skills to speak with patients and providers
- Understanding of HIPAA privacy laws
- Provide education regarding chronic medical conditions to patients
- Perform patient assessments and basic nursing functions
- Monitor and analyze patient vitals data
- Understanding of virtual patient meeting etiquette
- Proficiency utilizing Windows-based computer applications, email, and web-based apps
- Spanish speaking is a plus
Licenses and Certifications - Nursing license in the state of Florida Required
- Multistate Compact Nursing License is required (takes about 2 weeks to obtain)
- Additional nursing licensing in non compact nursing states is a plus
