JOB SUMMARY:

Goodwin Home Health and Goodwin Palliative Care and Hospice is seeking an articulate, motivated, and high-energy individual with experience in home health marketing strategy who will ensure comprehensive community outreach about home health and hospice services. The Clinical Liaison develops relationships with case management staff and others at hospitals and other facilities to ease discharge planning and encourage admission to our hospice or home health team. The Clinical Liaison is responsible for patient assessments and admissions, ensuring continuity of care between the admitting process and transition to ongoing care by the home health/hospice interdisciplinary team (IDT).

DUTIES:

  1. Is responsible for a well-organized approach to effective discharge planning so patients return home with the appropriate amount of resources to meet their goals of care.
  2. Is able to recognize and work within each hospital/facility culture to effectively meet the needs of the case management staff.
  3. Builds relationships with Hospice’s vendors so that DME, medication and supplies are delivered promptly to the patient’s homes.
  4. Educates health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice or home health care.
  5. Is proactive in identifying new referral sources, increasing referrals from existing sources, and providing educational programs to hospitals and facilities. 
  6. Maintains good communication with home health and hospice clinical staff in order to enhance services, and facilitates communication between IDT and referral sources.
  7. Promotes exemplary customer service to physicians, facility personnel, client and client family at all times.
  8. Works with home health and hospice administrators and Admissions Manager to identify referral trends and suggests ways to strategically build referrals and improve hospice admission processes.

 

QUALIFICATIONS:

  1. As a liaison with the community, excels and models professional and customer-service oriented values representing the mission and vision of Goodwin Living.
  2. Licensed Virginia discipline preferred.
  3. At least one year of recent marketing experience in home health/hospice; with at least three years total of marketing in the health care industry.
  4. Proven ability to develop and implement a sales and marketing plan.
  5. Evidence of achieving referral goals within the market.
  6. Excellent planning, organization and presentation skills are critical.
  7. Outstanding customer service, verbal, and written skills required.
  8. Able to work independently and in a collaborative team environment.
  9. Able to perform detailed work with a high degree of accuracy.
  10. Able to maintain confidentiality of data.
  11. Demonstrated computer skills for documents, spreadsheets and database management and ability to learn new electronic medical record software.
