Overview This is an Entry-Level Position
As a member of the Population Health Management (PHM) Team, the Outpatient Case Management
Project Assistant will work closely with the leadership and case managers in a supportive role.
Performs clerical duties which includes, setting up and preparing for meetings, taking meeting minutes, creating PowerPoints and other documents using Microsoft office programs. Will be responsible for setting up IT access and onboarding documents of all new staff. Orders all office supplies and manages invoices for payment. Will be the timekeeper and manage kronos.
Will assist the case managers to identify and coordinate start to finish resources necessary to help patients manage their healthcare needs in the community. Will assist with identifying primary care physicians for unassigned patients, set up and schedule follow up visits, ensure travel arrangements are made and in place and set up medication assistance programs. Will track and manage work list and files for case mangers as needed. Will track SNF discharges and refer to OP case managers timely. Under direction of the Case Manager, assists with routine referrals for Post-Acute Care Services as well as schedule all necessary appointments. Will work on care gap closure and onboarding of new member to the PHM programs. Will work in a supportive role to assist case managers with making timely follow up phone calls to patients.
Performs other duties as assigned.
The goal of the PHM OP Case Management team is to effectively manage patients on an outpatient basis and during episodes of acute hospitalizations to assure the appropriate level-of-care is provided, to prevent inpatient re-admissions and ensure that the patients' medical, environmental and psychosocial needs are met over the continuum of care.
Responsibilities This is an Entry-Level Position
Core Responsibilites and Essential Functions
- Community Resources and Coordinating with Case Mangers
* Reviews all patient referrals and determine priorities.
* Performs comprehensive assessment of available resources to assist patient/family with needs.
* Reviews all options/resources available to meet client/family needs and coordinate the services for patients.
* Assist case managers with follow up phone calls for simple reminders to patient/family.
* Collaborates with physician?s office practice as needed to coordinate care.
* Coordinate patient discharges and ensure that patients are adequately distributes to OP CM?s.
* Track SNF admissions and discharges and ensure timely referral to CM?s. - Key Clerical Duties
* Managing meetings, creating meeting documents.
* Set up IT access and onboard all new staff on systems.
* Managing kronos.
* Assist with tracking employee time off.
* Orders all office supplies and distribute to staff. - Key Role/ Monitoring/Evaluation
* Monitors services through data collection and analysis. Evaluates processes utilizing a systematic approach to determine the effectiveness of the services.
* Reviews results with population health management team and continually work to improve identified gaps.
* Participates in team meetings to evaluate current processes, provide and receive feedback, review specific cases with goal of problem-solving for improved patient adherence to plan and services set up.

Qualifications This is an Entry-Level Position
Required Minimum Education
- Associate's Degree Required or
- Bachelor's Degree in a related field highly preferred OR an equivalent combination of education and experience.

Required Minimum Experience
- Minimum 2 years customer service or experience as office assistant. Required and
- Experience working with data, conducting simple analysis and maintaining spreadsheets Required and
