Position Summary: Work with the Director of the Long-Term Care Ombudsman Program and the State Ombudsman Office to manage the responsibilities of the program and to enhance the quality of life of assisted living residents by advocating for the residents of assigned facilities. Serving as a link between residents, families, staff, and others to resolve resident complaints and concerns.
Essential Functions:
- Advocate for the residents in the assisted livings within the catchment area of the LTCOP of AgeSpan (which also includes Beverly, Essex, Gloucester, Hamilton, Ipswich, Lynn, Lynnfield, Manchester-by-the-Sea, Nahant, Rockport, Sagus, Swampscott, Topsfield, and Wenham).
- To assess, resolve and provide information and referral for concerns of residents, families and the community that are received during a facility visit or through the intake system.
- To complete quarterly visits to assigned facilities (up to Approximately 61 facilities), until a time when weekly visits can be made, and otherwise authorized by the State Ombudsman Office.
- Consult with the State Assisted Living Ombudsman for concerns that occur during visits or from the phone intake system.
- Maintain open communication during visits with assigned facility contact person to resolve concerns of residents brought forward during visits.
- Identify and report issues that may affect the health, safety, welfare, and resident rights and or any serious concern, including reports or observations of abuse, neglect, or mistreatment immediately to the State Ombudsman.
- Maintain confidential information regarding the residents, their concerns only to be shared within the Ombudsman Program and with consent of the resident to all others.
- Notify the Program Director and State Ombudsman immediately of any conflicts of interest that may arise during time as serving as an Ombudsman.
- To be knowledgeable about federal and state regulations that affects the quality of life of residents.
- Consult with Facilities as requested to address concerns and needs of residents.
- Maintain case records, provide primary coverage for phone intakes, intake logs, compile facility survey data, enter data information and perform other administrative functions as needed.
- Complete visit reporting requirements within 48 hours of completion of a visit, in accordance with the reporting policies for visits and case documentation.
- Provide coverage for Facilities when a Volunteer Ombudsman is not available.
- Provide support to the ALR regional network in conjunction with or in the absence of the State Assisted Living Ombudsman as assigned.
- Develop Volunteer recruitment materials both printed and virtual.
Additional Functions/Requirements:
- Successfully complete a certification training presented by the Massachusetts Office of the State Long-Term Care Ombudsman.
- Attend monthly State Ombudsman meetings.
- Complete re-certification training every two years, regardless of when first certified.
- Complete 24 hours of continuing education per year.
- Provide in Field Supervision with the Volunteers as required.
- Attend the monthly All Staff Meetings and LTCOP Team Meetings as well as other agency related trainings and activities.
- Provide Program Director with State approved reports for monthly reporting to the agency.
- Complete additional tasks and duties as assigned by Supervisor.
- Bi-Lingual English/Spanish Preferred but not required.
Qualifications: Bachelor's Degree in Health and Human Services or related field and at least three years of prior work experience with elders in community, Long-Term Care settings, and/or with diverse, disadvantaged adults or currently pursuing a degree in human, health, advocacy, or legal services. Knowledge of the Assisted Living Industry, excellent communication, attention to detail, coordination skills, computer skills, and highly organized with the ability to be flexible. Can establish and maintain ongoing relationships with both individuals and organizations.
Become certified as an Ombudsman within 60 days of employment.
