Overview

You will apprentice under the supervision of our experienced CWCN team at DHMC.

Certified Wound Care Nurses (CWCN) are specialists who provide treatment for patients with wounds including, but not limited to, non-healing surgical wounds, diabetic foot ulcers, pressure injuries, venous leg ulcers, arterial ulcers and more. The certified wound care nurse has the responsibility and accountability for assessing and monitoring wounds, recommending dressings and plans of care to assist with wound healing and preventing skin breakdown. They work collaboratively with other members of the patient care team to determine if additional treatments are required. Educating patients and caretakers on wound care, infection and injury prevention is also a critical role. The certified wound care nurse provides direct patient care to patients and provides for ongoing staff development in wound care. This specialty requires a combination of patience, empathy and ambition in order to stay up-to-date on best practices and to help patients overcome physical and mental hurdles.

You would be working as a Nurse Clinician in the outpatient Comprehensive Wound Healing Center at DHMC in Lebanon with a team comprised of Wound Care Certified APRNs, Nurses, Podiatrists and LNAs.

Coordinates the interdisciplinary care of their patients. Ensures appropriate patient care and family involvement and education and school/community consultation.

Responsibilities

  1. Supports patient and families through procedures, medication therapy, emotional and financial issues, etc.
  2. Provides telephone triage, prescription needs, and notification and implication of lab results.
  3. Arranges for lab work and procedures. Provides timely notification to families of the results of test procedures, radiology studies, blood work, and any other tests ordered by the team.
  4. Assists with patient and family needs and patient education materials.
  5. Documents patient problems and progress.
  6. Participates in interviewing and evaluation of staff.
  7. Works with Nurse Manager to ensure adequate nursing coverage.
  8. Acts as multi-disciplinary liaison with staff from radiology, laboratory, financial services, pharmacies, medical insurance companies, referring providers, and the in-patient units.
  9. Assists with training and education of nursing students, medical students, and medical residents.
  10. Participates in relevant educational offerings to enhance effectiveness and achieve required in-service training, and certification.
  11. Performs other duties as requested or assigned.
