Job description
Position Summary : The FamilyDirected Services Program Manager will evaluate the need for services prioritize the level of services needed and document outcomes. Responsible for accurate documentation understanding of the financial connection between services and budgets and evaluating the appropriateness of proposed services in meeting the essential needs of the participants in the FamilyDirected Services (FDS) Program. Evaluate communicate and oversee staffing needs and development for direct support staff professionals and personal care specialists in HeM 521 and HeM 525 programs ensuring quality services high performance and continuous program improvement.
Essential Responsibilities :
Review and understand all rules and regulations promulgated by the New Hampshire Bureau of Developmental Services (BDS) related to FDS.
Complete and track Certification and/or Prior Authorization renewal dates for each participant utilizing Participant Directed and Managed Services (PDMS) (525) or Personal Care Services (PCS) (521)
Prepare and present information during transition meetings to assist families in planning for adult services in PDMS and PCS; create budgets and proposals as necessary
Complete certification packets; submit to Bureau Liaison within appropriate timelines to ensure no lapse in certification or funding
Ensure that budget matches proposed services and participant needs and meets applicable regulations (e.g. HeM)
Generate review and monitor contracts as appropriate
Facilitate payment for services as needed troubleshoot when necessary
Communicate regularly with participants and teams members regarding service changes etc.
Ensure compliance with BDS and Bureau of Elderly and Adult Services regulations including waivers
Generate and monitor all tracker budgets including changes/amendments etc.; ensure proper documentation is obtained (doctors notes Medicaid denials etc.) and all caps are being met
Qualifications :
Education and Experience
Bachelor s degree in Human Services or related field preferred or a combination of related postsecondary education plus a minimum of two years related work experience (consideration may be given for four years of comparable experience working with people with disabilities or behavioral health or clinical experience in lieu of degree)
Previous experience developing community relations and community resources
Proficiency in Microsoft Office Suite and database systems
Valid driver s license and access to a safe operating vehicle during scheduled working hours
