Job description

Join our team of dynamic and driven individuals as a Supports Broker, where you will play a crucial role in helping Participants achieve their full potential through a self-directed employer service model. The Support Broker will assess Participants' abilities and work closely with authorized representatives to ensure their successful participation. Identifying community resources and making need-based, person-centered referrals will support Participants in reaching their goals and realizing their dreams!

Key Responsibilities:
- Evaluate Participants' and authorized representatives' potential for success in a self-directed service model
- Offer valuable insights and recommendations for placement, training, and improvement
- Stay in constant communication with Participants and authorized representatives to provide the necessary support and accommodations
- Build and maintain an extensive network of community resources to meet Participants' needs
- Provide follow-up support to ensure success and track outcomes after referrals
- Keep an updated internal resource guidebook for assigned state regions
- Monitor referral requests and identify trends for improvement
- Expand the network of referral sources through research and networking efforts
- Deliver effective, person-centered education and training to Participants and authorized representatives
- Empower Participants and authorized representatives to act as household employers and self-direct their services
- Continuously evaluate and improve the performance of authorized representatives
- Conduct regular in-person visits and telephone calls with Participants, authorized representatives, and Providers
- Offer hands-on support for enrollment activities and paperwork
- Facilitate the development of tailored service and spending plans
- Report any suspected fraud, abuse, neglect, or exploitation
- Document services provided and time committed accurately and efficiently

Requirements:
- A Bachelor's degree in a related field is preferred, with at least one year of related experience
- Must be a NJ resident in Somerset, Mercer, Passaic, Bergen, Hudson, or Warren County.
- Excellent decision-making skills and the ability to assess Participants' needs and abilities discreetly
- Strong verbal and written communication skills
- Ability to understand and support self-directed services
- Ability to prioritize, manage multiple tasks, and meet deadlines
- Maintains confidentiality in the work setting
- Proficiency in Microsoft Office software, CRM, and Web Based applications
- Establish and maintain positive relationships with agencies and community stakeholders
- Ability to generate required service documentation in an accurate and timely manner
- This is a remote position with regular participant site visits. A valid driver's license is required, with a mileage stipend provided.
