Position Summary:
Virtual Health Nursing is a conventional approach to support staffing, by utilizing IT, EMR and Telecommunication systems. This position is to develop the Virtual Registered Nursing pilot. The Virtual Registered Nurse is a member of the Nursing Care Team and is responsible for collaboration, communication and facilitation of optimal provision of care through the use of technology/digital platform supporting daily management of care. Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge. Utilizes clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care. Ensures a seamless, effective, efficient transition of care across the continuum. Responsible for monitoring quality metrics specific to the department & reviewing patient EMR for complete & accurate documentation in the patient record. Will provide bedside team members with positive culture coaching when appropriate based on chart review findings.
Qualifications:
EDUCATIONAL REQUIREMENTS Associate's Degree in Nursing. BSN preferred.
CERTIFICATION & LICENSURE REQUIREMENTS Registered Professional Nurse (RN) License Illinois upon hire and National Nursing Certification within 1 year.
EXPERIENCE REQUIREMENTS Three (3) years of recent inpatient bedside nursing experience.
ADDITIONAL REQUIREMENTS - Nursing Ladder Level 3 or higher
SKILLS AND KNOWLEDGE Proficient in Epic EMR.Ability to partner with operational/clinical leadership and bedside team members to effectively and efficiently deliver virtual health solutions.Writes, reads, comprehends and speaks fluent English.Proficient computer knowledge using word processing, spreadsheet, email, and web browser. Multicultural sensitivity.Critical thinking skills using independent judgement in making decisions.
Essential Functions:
- Uses technology to provide remote nursing care to an assigned group of patients in collaboration with the bedside nurse and multidisciplinary teamEffectively manage patient care by performing duties such as admissions/discharges/med history/etcDemonstrate advanced competencies and critical thinking skills in day to day practiceIdentifies and utilizes team resources to solve identified patient needs
- Displays positive communication interactions with patient, family, nursing, physiciansServes as an expert knowledge resource for Novice Nursing team membersContributes to the overall quality of patient carePositively contributes to the unit's culture by working collaboratively with bedside team members
- Ensures timely response to patient's needs and questionsDocuments and communicates collaboratively with the bedside nurse regarding the patient condition and all nursing care providedCollaborates with the bedside nurse to initiate, update, evaluate and revise the plan of care to facilitate the achievement of patient goals and expected outcomes
- Promotes optimum communication by answering patient/family questions, providing plan of care updates and facilitating family meetings as neededIn collaboration with bedside nurse, prepares the patient and family for discharge by providing discharge teaching and reviewing discharge instructions and recommended follow-up
- Assesses and coordinates patient's discharge planning needs with members of the healthcare teamIn collaboration with the bedside nurse and Care Management Team, participates in rounding, assists with discharge planning, and identification of post-discharge needs, including discharge disposition, to facilitate the timely transition to the next level of care for the patientsProvides coaching, teaching, educational materials and clinical support to bedside nurses.
