Job description
Northern Light Home Care and Hospice is requesting an experienced per diem, palliative care/hospice nurse practitioner for the greater Bangor area in Maine. This position would consist mostly of palliative care but would include some hospice, with a mixture of remote and community-based home visits. We are looking for that special someone who could commit to several hours per month but be flexible as needed. Here at HCH, we actively assist clients and families to achieve their highest level of personal health and independence. The right candidate would focus on clarification of patient and family goals of care, patient safety, appropriate coordination of care and patient self-management opportunities, all while delivering compassionate, professional, and high-quality care.
