Job description
Healthonomic Primary Care is a reputable medical group located in Evergreen Park, IL, dedicated to providing comprehensive preventive care services and managing chronic diseases. We are currently seeking a motivated and compassionate Nurse Practitioner to join our team. The ideal candidate should possess excellent clinical skills, bilingual proficiency, a positive attitude, and a strong ability to work collaboratively within a team setting.
Responsibilities:
Performing Initial Examinations: Conduct comprehensive physical exams and thorough patient observations to assess their overall health status.
Managing Chronic Conditions: Develop and implement treatment plans for patients with chronic diseases, ensuring effective disease management and promoting optimal patient outcomes.
Ordering and Administering Diagnostic Tests: Evaluate patient needs and order appropriate diagnostic tests to aid in accurate diagnoses. Administer tests when necessary and interpret the results.
Communicating Test Results: Effectively communicate diagnostic test results to patients and their families, ensuring a clear understanding of the findings, potential implications, and recommended next steps.
Recording Patient Medical Histories: Maintain accurate and up-to-date patient medical records, including comprehensive medical histories, examination findings, test results, and treatment plans.
Quality Measures Maintenance: Adhere to established quality measures and guidelines, ensuring compliance with regulatory standards and optimizing patient care outcomes.
Bilingual Proficiency: Utilize bilingual skills to communicate effectively with diverse patient populations and ensure an accurate understanding of medical information.
Positive Attitude: Foster a positive and empathetic environment for patients, demonstrating a compassionate and caring approach to healthcare delivery.
Team Player: Collaborate with physicians, nurses, and other healthcare professionals to provide coordinated and integrated care to patients. Actively participate in interdisciplinary team meetings and contribute to the development of comprehensive care plans.
Requirements:
Current unrestricted license as a Nurse Practitioner in the state of Illinois.
Master's degree in Nursing or related field.
Certification as a Family Nurse Practitioner or in a relevant specialty.
Proficiency in English.
Excellent clinical and diagnostic skills.
Strong interpersonal and communication skills.
Ability to work effectively within a team-based care model.
Knowledge of preventive care guidelines and chronic disease management.
Proficiency in medical record documentation and electronic health record systems.
