Job description
Schedule: Tuesday, Wednesday, Thursday 9am - 5pm
General Summary: Under supervision by clinical pharmacist(s), and working as part of an inter-disciplinary team, assists the team to help reduce medical and medication cost and improve patient outcomes. The pharmacy intern will provide a variety of services to accomplish these tasks, including: providing patient and provider education, conducting medication therapy management and medication reviews, performing drug information searches and follow-up recommendations, making recommendations for appropriate drug therapy according to evidence-based medicine, working to close gaps in care, providing direct patient care through services such as point of care testing, immunizations, and monitoring chronic conditions. The pharmacy intern will be expected to help document their services and report the outcomes to stakeholders.
The pharmacy intern will serve as an ambassador of Toledo Clinic and partners in the Northwest Ohio Accountable Care Organization (NWOACO) in the community.
Principal Duties & Responsibilities: - Conducting and documenting medication therapy management (MTM) sessions under pharmacist supervision
- Providing patient counseling under pharmacist supervision
- Identifying resources for patients to aid in affordability of medication
- Data collection of pharmacist or pharmacy intern interventions
- Conducting medication reconciliation with patients or caregivers upon transitions of care
- Preparing patient medication lists
- Providing recommendations to providers to improve medication therapy or medication regimens
- Updating medication list in the electronic health record
- Scheduling patients for appointments
- Documenting communication or recommendations in the electronic health record
- Making recommendations for over-the-counter products under the supervision of a pharmacist
- Monitoring medication therapies through performing profile reviews, interpreting laboratory values, and ensuring drug regimens are consistent with medication use guidelines and protocols
- Assessing patient’s progression to achieving desired medication treatment goals
- Promoting health, wellness, and disease prevention
- Establishing and interpreting patient information – active problem list, past medical history, physical exam data, laboratory data, hospital course, medication history, medication profile, pharmacokinetic evaluation
- Developing and recommending appropriate therapeutic plans, identifying, and resolving drug-related problems, communicating to other health care team members, following through, and documenting in the electronic record
- Reviewing medication orders for appropriate dose, duration, frequency, dosage form, indication and drug interactions
- Reporting weekly data on productivity
Other Essential Duties May Include : Duties of the Pharmacy Intern may include, but are not limited to the following:
- Providing patient care over the phone, in person, and virtually
- Calling insurance companies to inquire about prescription drug coverage for patients
- Creating reports and delivering presentations summarizing interventions and outcomes
- Assisting pharmacist in performing collaborative practice agreement tasks
- Assisting with initiatives and patient care programs hosted by the Toledo Clinic (health screenings, immunization clinics, etc.)
- Providing all immunizations approved by the Ohio State Board of Pharmacy under the supervision of a pharmacist
- Calling to clarify medication orders
- Preparing or presenting results of interventions in scholarly venue or publication
- Completing pillbox fills for patients
- Retrieving, evaluating, and providing drug information to health care providers, answering general and patient specific drug information questions
- Reviewing documented adverse drug reactions/allergies and reporting new occurrences
- Completing quality improvement projects
- Other duties as assigned by the supervising pharmacist
Knowledge, Skills & Abilities: Required: - Establishes and monitors goals within the clinic and departments.
- Proficient in Microsoft Office, Microsoft Excel
- Experience with an electronic medical record or pharmacy dispensing system
- Computer savvy
- Basic Life Support Certification, in active status
- Ohio State Board of Pharmacy Intern License in good standing
- American Pharmacists Association (APhA) Immunization Certificate or other Ohio State Board of Pharmacy approved immunization training
- Experience drug databases, such as Lexicomp, Micromedex, etc.
- Commitment to working throughout the academic year and on a regular basis, including summers and extended breaks
- Highly motivated to help expand pharmacy services and dedicated to providing optimal patient care
- Exhibit exemplary customer service
- Present in a professional demeanor and dress
- Adheres to all Toledo Clinic policies and procedures as well as federal HIPAA, Fraud Waste and Abuse regulations and Ohio/Federal Laws.
Education : - Currently pursuing a pharmacy degree at an accredited college of pharmacy and in good academic standing.
- Enrolled in the professional portion of the pharmacy program
Preferred: - Experience providing Medication Therapy Management (MTM)
- APhA Medication Therapy Management Certificate or other formal MTM training
- Pharmacy student in second or third professional year
