Job description
BAYADA Hospice is looking for a compassionate and dedicated Nurse Practitioner to join our Fayetteville Hospice team per diem .
This office services hospice clients on a per visit basis in territories throughout Bladen, Cumberland, Harnett, Hoke, Moore, Robeson, and Sampson Counties .

As a hospice NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.
• You will be working with the office team to schedule and complete face to face visits, support with referral/admission review for eligibility and orders.
• 2 weeks/month rotation of on call to support the medical director, attend IDG meetings as appropriate, all staff meetings and training.
Prior home care and/or hospice experience strongly preferred, but not required.

Nurse Practitioner (NP) Responsibilities:
Follow a designated care plan in accordance with patient's needs
Make home visits to clients in designated geographic territories
Perform assigned duties, including administration of medication, wound care, treatments, and procedures
Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
Follow up with, execute, and properly document doctors' orders
Perform client assessments as necessary
Case management and coordination
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
Qualifications for a Nurse Practitioner (NP):
A current license as a Nurse Practitioner in the state of North Carolina .
Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases as assigned.
