What's the role?
Cayuga Medical Associates PC is looking for a social worker or mental health counselor to join our team as a Behavioral Health Care Manager in Ithaca, NY. The Behavioral Health Care Manager (BCHM) serves as a vital member of the Cayuga Integrated Behavioral Health team, an innovative and growing program within Cayuga Medical Associates in beautiful Ithaca, NY. The BHCM will work in close collaboration with other members of the CIBH team to expand access to high-quality behavioral health services to CMA's primary care population, which blends Primary Care Behavioral Health (PCBH) and Collaborative Care Model (CoCM) approaches. We are looking for individuals with a passion for caring for others and a willingness to be creative in an emerging program that supports our community's behavioral health needs.
What you'll do:
- The BHCM will provide mental and behavioral health care management services to primary care providers and their patient populations, to include: mental health screening, assessment, brief intervention, and patient engagement/tracking services.
- The BHCM will work in close collaboration with an interdisciplinary team including physicians, nurse practitioners, physician assistants, registered nurses, behavioral health consultants, and consulting psychiatrists to provide comprehensive care management support to patients with commonly occurring mental health concerns, namely depression and anxiety.
- The BHCM is responsible for identifying patients appropriate for services, conducting initial mental health assessments, assigning patients to a caseload/registry, providing brief intervention and care management services, and working closely with the consulting psychiatrist and primary care provider to systematically track patient outcomes and coordinate care.
- The BHCM provides psychoeducation on depression and anxiety, offers brief evidence-based interventions (e.g., motivational interviewing, behavioral activation, problem-solving treatment) to patients under their care, and supports referrals to community services and higher levels of care when indicated.
- In addition, the BHCM supports psychotropic medication management as prescribed by medical providers, focusing on monitoring treatment adherence, side effects, and effectiveness of treatment.
- The BHCM participates in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient's medical provider.
- The BHCM tracks patient follow up and clinical outcomes using a registry and documents care accordingly in the patient's medical record.
- The BHCM develops and implements relapse prevention self-management plans with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
- The BHCM actively and proactively participates in quality-improvement initiatives and organizational working groups as assigned.
- Once trained and experienced in the care model, the BHCM will assist in training and onboarding new behavioral health professionals to the care team.

We are looking for a Behavioral Health Care Manager to work in close concert with patients, a psychiatric consultant, and the larger primary care team using standardized tools and evidence-based practices to help meet defined patient care goals. The ideal candidate for this position will meet the following requirements:
- Training in one of the following fields (current New York State licensure required):
- Social work, to include LMSW and LCSW
- Mental health counseling, to include LMHC
- Flexibility to adapt to fast-paced medical environments as a member of a dynamic care team.
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Ability to establish and maintain effective and professional relationships with patients and other members of the care team.
- Experience with assessment and treatment planning for common mental health and/or substance use disorders, as well as working knowledge of differential diagnosis for these conditions.
- Experience with and interest in providing care to pediatric-aged patients.
- Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
- Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of primary care.
- Ability to work effectively engage patients in a therapeutic relationship, especially via telephone and/or telehealth modalities, potentially from a remote environment.
- Proficiency with technology, including video conferencing platforms.
- Familiarity with using electronic medical record systems. Skill in concurrent documentation is a plus.
- Ability to organize and prioritize tasks for completion.
- Training and/or experience in integrated behavioral health care (especially PCBH and CoCM) is a plus, but training in these care models will be provided for candidates who do not have this specific training background.
