Job description

Scope of Position

Provides clinical assessments and treatment plans according to the established Outpatient Care East Cardiac and Pulmonary Rehab protocols and guidelines for prevention/wellness and chronic disease management programs. Plans, implements, evaluates, and modifies programs for participants in outpatient rehab. Preforms EKG stress testing on Cardiac Rehab patients and fire fighters with completion of testing documentation in the EMR. Educates program participants and staff in prevention and rehabilitation process to better understand participants condition and adopt a plan of care. Maintains appropriate follow-up of patients. Participates in performance initiatives. Manages patient admissions and charge entry, ensures cleanliness of the treatment space. Communicates with patients, families, management, staff, and physicians.

Position Summary

Provides direct patient education and care for outpatients of Outpatient Care East Cardiac and Pulmonary Rehabilitation Programs. Assists in clinical, technical, clerical, and educational operations at OSU Outpatient Care East. Must display highly refined skills in interpersonal communication, organization, program planning, and customer service.

65% of the time Coordinates, executes, and maintains quality patient care in Cardiac and Pulmonary Rehabilitation programs

Performs clinical assessments according to established protocols and guidelines for cardiac rehabilitation:

Performs and assists in exercise stress testing (EKG stress tests, 6 minute walks, etc) at OSU Outpatient Care East as well as needed documentation for testing billing and EMR data entry. Assists in planning, implementing, and evaluating programs for preventative, rehabilitative and lifestyle modifications for participants. Provides and or assists with supervision during exercise sessions and strength and mobility training of participants of both Cardiac and Pulmonary Rehab patients. Provides and or assists with orientation and continuing in-service training programs for staff. Develops and or assists with individual health and maintenance care plans for participants which includes weight management, smoking cessation, and other lifestyle factors. Monitors and reports achievement rates, quality measurements and customer satisfaction levels. Teaches and develops educational classes for participants in support of rehab protocols. Provides general coverage of the exercise floor, core classes and other operational areas of the clinic Manages medical records Documents assessments, patient progress and patient clinical issues in chart and to physician Problem solves and facilitates communication regarding billing problems

35% of time Participates in organizational responsibilities and program development

Responsible for case management for the cardiac rehab patients, coordinating benefits and scheduling.

Develops forms (electronic if appropriate) and processes to expedite documentation and assure compliance to practice standards. Provides support to all other health and wellness programs and activities, e.g., special populations, special events, conferences and general consumer classes at the Medical Center Initiates and participates in performance improvement initiatives, program outcomes, and other compliance programs to meet JCAHO standards. Participates in JCAHO and NCQA accreditation and compliance surveys as appropriate. Maintains appropriate documentation for these surveys. Works with outpatient clinicians to ensure that ongoing rehab needs of OSU Outpatient Care East patients are met. Performs charting, data entry and analysis for organizational reports. Acts as liaison for outpatient Cardiac and Pulmonary rehabilitation. Participates in organizational meetings and communicates with other department managers and staff. Provides student and intern supervision for exercise physiology students. Provides orientation of new staff members and oversees documentation compliance. Assists in promoting positive customer relations and customer satisfaction for OSU Outpatient Care East/Medical Center Performs other duties as assigned and required to facilitate the efficient operation of the Cardiac and Pulmonary Rehab programs at OSU Outpatient Care East.

Will report immediately to management any observed breach of the Medical Centers policies and procedures or suspected ethical violations (especially fraud or abuse). Minimum Qualifications

Degree in health education, fitness, exercise science, or related field; experience in healthcare and /or risk reduction programs with clinical skill base and familiarity with telemetric monitoring required; experience in cardio/pulmonary rehabilitation preferred. ACSM certification preferred. ACLS certification required after hiring. Knowledge of computer office skills; exceptional customer service skills necessary.

Maintains ACLS certification; meets mandatory education and health requirements; demonstrates competence in technical, interpersonal, and cognitive skills required to meet essential job functions.

Job Relationships

Supervisory Responsibility: Exercise physiology students and interns

Contacts: Interacts daily with patients, families, caregivers, physicians and their supportive staff, nurses and their support staff, PCRMs, other team members (inpatient and outpatient). Interacts on a weekly basis with referral sources and payers, clerical staff, managers and administrators, admissions personnel, visitors, and vendors. .

Work Environment

The work environment involves everyday risks or discomforts, which requires normal safety precautions typical of such places as offices and meeting rooms (i.e. use of safe work practices, avoidance of trips and falls, and observance of fire regulations and traffic signs). The work environment involves risks associated with the use of computers such as eye strain. The hepatitis vaccination is offered to employees as a preventative measure.
