OUR HOUSING TEAM IS GROWING! Be a part of this growth and help make a real difference in the lives of our consumers.
OCES is seeking a caring professional who enjoys working in the community to work with our Housing team to ensure services and support to residents on-site in a new Brockton area location. Bi-lingual - Haitian Creole required.
What you will do:
- Review the needs of residents in order to link to community programs and services.
- Serve as the Multi-disciplinary Assessment Team Leader and meet with the team to assure service coordination.
- Facilitate care conferences with residents, family, and other participants.
- Plan and coordinate social, educational, and recreational programs for residents.
- Prepare and distribute a newsletter to keep residents informed of planned activities.
What you'll bring:
- Your passion for helping others and desire to make a difference
- Adaptability to be able to change as the needs of consumers and/or programs change
- At least two years of experience in the human services field preferably with the elder population
- Bachelor's degree in Social Work or related field preferred
- Bi-lingual - Haitian Creole required!
