CareAparent, a Medicare-Certified Home Health agency accredited by the Joint Commission, is seeking new graduates and experienced (LPN) Licensed Practical Nurses to serve our aging population throughout the Saint Cloud area in continuing to live independently and safely in their homes for as long as possible. As an LPN, you will conduct in-home visits, aide in preparation of care plans, perform medication set-ups, supervise Caregivers and CNAs, and assist with caregiver training.
Further duties include:
- Performing skilled care functions in the home, including medication management, medication set up, treatments, and procedures
- Documenting visits, orders, actions, and conditions
- Monitoring client conditions, reporting changes to Clinical Manager or DON
- Performing client assessments as necessary
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet

If YOU have the following, CareAparent wants to hear from you:
- Current Minnesota license as a Licensed Practice Nurse.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Ability to work on-call for rotating evenings, weekends, and/or holidays, as required.
- Strong interpersonal skills.
- Excellent verbal and written communication skills.
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
- Valid driver's license, reliable transportation, and ability to travel to cases as assigned.
