UPMC’s Internal Medicine of Camp Hill is looking to add a Professional Staff Nurse (RN) to their team. Our practice has 7 providers and is a family atmosphere with long time patients. We are an Internal Medicine office with patients who are 18 and older. The RN is an essential member of our team, managing and providing patient care through the application of independent judgment, communication and collaboration with all team members. We have a large panel of geriatric patients with multiple diagnosis. We are looking for someone that can work independent, having their own patient schedule and complete Medical Wellness Visits. You would also be responsible for quality metrics in our office and will occasionally have incentives/earn rewards for working quality reports. You must have experience with Excel and knowledge of how to work with spreadsheets to organize your quality metrics.
HOURS: Monday-Friday 8am-4:30pm, NO WEEKENDS, NO HOLIDAYS, NO ON-CALL
Responsibilities:
- Quality – Making sure patients are up to date with their quality metrics (Mammograms, Dexa Scans, Medicare Wellness Exams, (Eye Exams, A1C, Microalbumin for diabetics), etc
- Managing your own patient schedule for Medical Wellness Visits and keep your schedule filled 4 days a week.
- Manage Coumadin Clinic and the PT/INR’s bloodwork results – coordinate with the provider necessary directions for blood thinner prescriptions according to their INR results and contacting patients.
- Managing your ‘In Basket’ of messages, triaging and making appointments if necessary
- Work various reports for Highmark STARS, True Performance, and Not Meeting; calling patients to set up appointments to ensure all their quality is up to date
- Provide backup to our MA’s by running providers and doing vital signs when needed.
- Actively participates in department or practice-specific quality improvement efforts. Identifies opportunity for quality improvement to colleagues and management.
- Takes personal responsibility in improving patient satisfaction with the quality of care and service.
- Utilizes research and evidence-based practice to support improvement in clinical care and identifies research issues or articles related to clinical specialty or areas of interest.
- Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
- Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment.
- Good clinical judgment with critical thinking, analytical and problem-solving abilities required as related to various aspects of patient care.
- Critical thinking skills necessary to exercise and to lead others in application of the nursing process.
- Demonstrates knowledge of adult learning principles (and/or teaching children if applicable) and applies in teaching of patients, families, students and new staff.
- Provides detailed and appropriate teaching to patients and families to effectively guide them through the episode of care both in the office setting and via phone/written communication when at home.
- Formulates goals and a plan of care for patients that involves the patient as partner and considers the individual needs of the patient in a holistic manner.
- Patient documentation is comprehensive and promotes communication between caregivers.
- Incorporates national professional organization as well as business unit and health system’s goals to improve patient safety, quality and satisfaction.
