We have an exciting opportunity for a full-time (35 hours per week) Case Manager in our Guardianship Program. The hours are Monday through Friday generally between 9:00 a.m. - 5:00 p.m. We are offering flexible hours with a hybrid remote option.
Our corporate office is located in Fall River- However, the catchment area for this role will be in Merrimack Valley in Massachusetts. This position will be hybrid remote but, would need to come to the office in Fall River 1-2 days per month and during the training period.
The Case Manager will be responsible for all case management duties including but not limited to the following:
Duties and Responsibilities:
- Regular face-to-face visits with assigned clients at their place of residence within the state of Massachusetts, including completion of monthly Progress notes, and documentation of collateral contacts.
- Case Manager will be required to provide his/her own reliable transportation for client visits. Occasional travel to Fall River for supervision or staff meetings may be required.
- All case management duties including, but not limited to, hospital admissions and discharges, placements at nursing homes and other facilities; obtaining legal paperwork such as Medical Certificates; setting up funeral arrangements, managing issues involving personal and real estate property, and monitoring of personal needs, etc.
- Document Units of service for billing on each client, and submit to the Case Manager Supervisor for review.
- Follow policies and procedures established by the Guardianship Program.
- Maintain community relations, i.e., Social Workers, Physicians, discharge planners, Psychiatrists, and Attorneys to promote program visibility.
- Provide backup for decision-making responsibilities for clients to Case Manager Supervisor, other Case Managers, and assigned student interns, and/or volunteers, as needed.
- Responsible for checking phone messages and email correspondence on a daily basis.
- Responsible for covering other case management cases when required.
- Responsible for submitting weekly schedules to the Case Manager Supervisor.
- Respect the confidential nature of all program services and communications therewith.
- Review and consent to medications and medical procedures.
- Responsible for making sure all appropriate consents have been given (verbal and/or written, if required) prior to the scheduled procedure, including any weekend procedures that may take place.
- Respond to reasonable expectations of the Agency and/or supervisors.
- Prepare Monitor’s Reports and Guardian Care Plan Reports in a timely manner as requested by the supervisor.
- Participate in On Call Rotation requiring case manager to respond to emergency situation during weeknight/weekend, if necessary.
- Document any communications in the electronic health records, ie. phone calls, received mail, and conversations in ongoing case notes, especially on EOEA cases.
- Responsible for working with the fiscal dept to ensure that clients on MassHealth stay within the income guidelines set forth by the state of Massachusetts.
- Responsible to be familiar with the health insurance for each assigned client.
- To complete MassHealth applications and redeterminations.
- Process necessary checks for each client in accordance with proper procedures.
- As part of the check payment process, check the balance of all accounts and manage bill payments to ensure the account is never overdrawn.
- Responsible for checking balances of all accounts on a regular basis to ensure client accounts are not over asset.
- Report overdrawn and over-asset accounts to the Guardianship/Rep Payee Financial Supervisor, take appropriate action to remedy problems, and ensure resolution within one month.
- Post applicable deposits to the appropriate bank account.
Education and Experience:
- Must be a Graduate of an accredited university with a BS or BA in Psychology, Social Work, Sociology, or a related field, or commensurate experience and prior experience working with disabled/elderly adults in a community and/or long-term care setting.
- Experience with fiscal accounting or bookkeeping preferred.
- Experience with Case management, scheduling and setting up doctor and follow-up appointments to ensure continuity of care.
- There may be times when asked to transport a client to an appointment, this is not often but may happen.
