JOB DESCRIPTION
Our Palliative Care providers manage chronic debilitating illnesses and endof life issues. Provide cross coverage asneeded/requested by nursing and medical staff. Round on assigned medical hospitalist patients and on patients asrequested by facility medical staff.
Core responsibilities:
- Conducts family meetings
- Performs consults and follow up visits as needed
- Create, direct, coordinate, and supervise the patient care plansdaily
- Conduct/order diagnostic tests, and analyze results and prescribeindicated treatments and therapies
- Round on existing hospitalist patients in multidisciplinary roundsif asked
- Work as part of an integrated team with care management, nursing,pharmacy, etc
- Assist in patient education and goal setting
- Participate and adhere to all quality metrics and goals as establishedand directed by Adfinitas or the client/hospital
- Submit accurate and timely charges for all professional servicesperformed
- Attend committee, CQI and management meetings, as appropriate

REQUIREMENTS - 1+ years of experience as anAdvanced Practice Provider in an acute role, preferably long-term care orskilled nursing facility or similar inpatient experience required
- Graduateof an accredited school and program, respective to role (PA-C, or NP)
- Licensureto practice medicine in good standing, with the state governing body
- FederalDEA certification in good standing
- StateCDS in good standing
- Possess strong clinical skills and demonstrate competency inproviding comprehensive health care including evaluation, diagnosis andtreatment
- Flexibility and organizational skills to work autonomously
- Proven skills, knowledgebase, and judgment necessary for independent clinical decision making
