Are you a compassionate and skilled registered nurse seeking a fulfilling role in palliative care? We have an exciting opportunity for a Remote Palliative Care Nurse to join our team. Key Responsibilities: Provide nursing services based on the patient's individualized care plan, observing and evaluating treatment effectiveness and the patient's physical/emotional response. Collaborate with interdisciplinary teams, actively participating in planning, case conferences, and staff meetings. Document patient assessments, treatments, and plans accurately, adhering to established guidelines. Engage in case management and care coordination activities to support patients' plans of care. Manage a designated Care Team, ensuring visit frequencies align with the patient's acuity level and needs. Educate patients and caregivers about our services and palliative care, providing information on specific disease processes, symptom management, and the importance of medication management. Essential Organizational Duties: Adhere to company policies and guidelines at all times. Utilize video conferencing as needed, demonstrating a willingness to embrace technology. Ensure effective resource utilization and contribute to maintaining positive relationships with all stakeholders. Respect confidentiality, maintaining HIPAA compliance in all interactions. Promote the company's philosophy of compassionate communication towards patients, relatives, visitors, and coworkers. Environmental Conditions and Physical Requirements: Deliver palliative care primarily in a home setting, with potential for virtual work. Occasionally engage in reaching, stooping, bending, kneeling, and lifting items weighing 50 pounds or less. May perform procedures exposing the nurse to blood, body fluids, or other potentially infectious materials. Regularly use a telephone and laptop. Minimum Qualifications: Current California Registered Nurse license. Minimum 2 years of nursing experience, preferably in palliative care, hospice, or home health. Two years of professional nursing experience within the previous three years. Demonstrated exemplary clinical, analytical, and documentation skills. Excellent oral and written communication skills. Active, licensed driver with a reliable, insured automobile. Comfortable with technology, including EMR, Google Suite, and other patient management tools. Preferred Qualifications: BSN. CHPN certification or equivalent.
