Job description

Position Summary

Patient Care Coordinators are responsible for providing excellent customer service and experiences to patients. Patient Care Coordinators will coordinate with team members and patients to ensure alignment with goals, maximize performances and treatment standards, and ensure compliance with scheduled visits.

Patient Care Coordinators will initiate converting referrals, collecting of co-pays and supply charges, and ensuring insurance authorization and benefits are handled appropriately.

The successful incumbent will possess strong relationship skills as well as the ability to work within a highly fast-paced and team-centric environment.

Objectives

· Effectively manage patient caseload

· Ensure accuracy of insurance authorization

· Effectively collect and enter data

· Plan and coordinate with site Director of Clinical Operations

· Be an active participant in all weekly and one on one meetings

· Plan and coordinate with Director of Communications

· Ensure all patient POC are being met weekly

· Analyze individual metrics to better serve our patients and collaborate with team members

· Track, measure, evaluate, and report on progress towards achieving targets while adjusting strategy as needed

· Initiatives are aligned and working synergistically to address individual patients needs

· Effectively direct and communicate support staff as needed

· Share priorities and roadblocks proactively with Director of Clinical Operations for additional guidance and support

· Know and honor company core values

· Be responsible for directly and indirectly interacting with site team and leadership

· Ensure that all improvements and changes with patients are aligned with their overall rehabilitation expectations and goals

Competencies

· Ensure smooth and timely patient flow

· Strategize, schedule and help carry out treatment plans

· Track and report treatment progress

· Assure patients are treated promptly in accordance with their scheduled appointments

· Analyze and organize the clinic’s operations

· Connect with patients and clinical team members within your organization

· Ability to interpret performance data to inform patients decisions

Education and Experience

· Previous experience in Customer Service and administrative roles

Physical Requirements

· Periods of prolonged sitting
