Position Summary:

Work at the direction of the Director of Therapy according to the scheduled duties. Ensures Bowes In Home Care (BIHC) is compliant with all applicable federal, state, local and Medicare laws in providing patient care and implementing treatment of care with speech, language and communication disorders in coordination with the patient’s doctor or podiatrist. Responsible in conducting necessary evaluation, treatment, assessment, follow-up and reevaluation. Ensure that all necessary documents are completed and place in a secured and safe place. Ensures all service provided is completed according to BIHC standards and expectations

Position Requirements:

Have successfully completed an approved Speech Therapy program and provide proof (copy of) valid Speech Language Pathologist license.
An individual with at least a year of experience in healthcare and caring for the elderly.
Copy of CPR certificate.
Valid Driver’s license and Insurance.
Be able to travel to do patient care.
Must successfully pass a criminal background check in accordance with the Health Care
Worker Background Check Act (225 ILCS 46) and Health Care Worker Background Check Code (77Illinois Administration Code 955).

Must submit to and have negative results in a drug and alcohol screen.
Provide proof of a recent TB test with positive reactors.
Provide a copy of a health exam by a licensed physician no less than twelve (12)
months prior to the date of hire.
Must be able to lift at least twenty-five (25) pounds.
Professional communicator, who can establish rapport with patients and deal with concerns and problems in a professional manner.
Must be eligible for employment in the United States.
Must have a professional and positive work behavior.
Accessible 24 hours a day, 7 days a week.
Performance Standards:

Involvement with training, shadowing, instructing and guiding others
Ensure that the Agency is compliant with all applicable federal, state, local and
Medicare laws and regulations.
Ensures that services provided through BIHC conform to the corporate bylaws, policy and procedures, and IDPH rules and regulations.
Review physician’s referral and patient’s clinical record (when available) to determine the need for speech therapy services.
Conduct initial evaluation, assessment, follow-up and reevaluation of patients with speech, hearing or language disorders and establish a plan of treatment.
Assist the physician in determining and recommending appropriate speech and hearing services as well as developing and renewing the physician’s plan of treatment.
Providing rehabilitative services for speech, hearing and language disorders according to the physician’s orders and rehabilitation goals.
Recording and reporting to the patient’s physician and the Director of Therapy, the patient’s reaction or progress to treatment and any changes in the patient’s condition
Provide instructions and coordinate with other health care team, patient and family members with the home rehabilitation program as well as providing written instructions when applicable to ensure continuum of care.
Participate and assist in the development, implementation, review and revision of the plan of treatment as a member of the home health team, documenting clinical and progress notes, summaries, and other documentation.
Preparation of progress notes, summary reports (every 60 days), medication profiles, discharge summaries, and other documentation to the Agency and the patient’s physician or podiatrist and ensure proper documentation and reports are done according to licensure and Agency guidelines.
Keep, maintain and update patient records, information, case assignments and its management in a safe and secure place.
Maintain a healthy and professional partnership with the patient and market our service from within and out of the community.
Be a resource and support person for the patient, family, aides and the Agency in
regards to care and services provided.
Makes supervisory visits to the patient no less than two weeks to ensure proper care and
services were given.
Maintain proper documentation and completion of appropriate records in a timely manner
as outlined by the Agency.
Participate in case conference, meetings, In Services and training programs as required by the Agency to ensure professional growth.
Be available during all hours of operation of Bowes In Home Care and participate in all activities relevant to the provision of home health services.
Perform any additional duties that meet local and state guidelines as determined by the Director of Therapy.
