The School Based Health Behavioral Health Coordinator is an integral member of the SBH team and works in cooperation with the Behavioral Health team members to schedule incoming referrals. The SBH Behavioral Health Coordinator assumes the responsibility for electronic medical records maintenance according to the departmental guidelines and ensures follow-up for referral appointments through communication with internal and external providers and SBH patients. Responsibilities include:
- Receive and track all incoming referrals for behavioral health services.
- Responsible for proper maintenance of all electronic medical record information related to the referral process, which may include, but is not limited to transferring, retrieving, creating, and repairing records.
- Assumes responsibility for correctly entering all required registration and insurance information on established and new patients into the computer. Also, periodically verifies above information to ensure accuracy.
- Schedules internal new appointments for SBHC patients. Ensures all required paperwork – registration, release of information, and consent forms are completed by guardian before appointment is scheduled.
- Assists providers in requesting patient records when needed.
- Provides follow-up information regarding the referral appointment to the referral source.
- Maintains a close working relationship with all BH clinical staff to ensure optimal patient flow.
- Protects the confidentiality of patients by complying with the BRH patient confidentiality policy.
- High school diploma or equivalent required.
- Licensed Practical Nurse or Medical Assistant with primary care/ambulatory care clinical experience preferred.
- Bilingual – Spanish language, preferred.
- Experience with electronic medical record systems and/or previous referral coordination preferred.
- Ability to organize, communicate and perform detailed functions required.
- Strong interpersonal skills
