That care is provided within the Agency's scope of services, policies and procedures, state and federal laws and regulations, the Nurse Practice Act and Accreditation entity requirements.
To be considered for this office/field position the following must be listed among minimum qualifications:
• Graduate of an accredited school of professional nursing, and;
• Currently licensed as a Registered Nurse in Indiana or holds a Nurse Licensure Compact (NLC) license recognized by the
State of Indiana, and;
• Has two (2) years of nursing experience, and
• Maintains CPR Certification.
• Ability to supervise and direct care team members.
• Knowledge of home health regulatory and reimbursement requirements.
• Home Care Experience preferred.
Essential functions include: • Available during regular business hours in person, by phone, or by other Agency approved method of communication.
• Performs comprehensive patient assessments and collects OASIS data, where applicable, for assigned patients according to Agency policy and State and/or Federal regulations.
• Coordinates and collaborates with Clinical Manager, Scheduling Coordinator, and Clinical Educator to facilitate staffing according to identified patient needs and skilled nursing and home health aide/CNS skill set.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s) in coordination with the attending physician, dentist, chiropractor, optometrist, or podiatrist.
• Provides, and/or coordinates for the provision of, services that are ordered by the physician as indicated in the plan of care.
• Reviews and revises the individualized plan of care, in coordination with the attending physician, dentist, chiropractor, optometrist, or podiatrist, as often as the severity of the patient's condition requires, but at least once every 60 days, or as required by Agency policy and State and/or Federal regulations.
• Develops a written summary report and ensures delivery to the attending physician, dentist, chiropractor, optometrist, or podiatrist at least every 60 days, or as required by Agency policy and State and/or Federal regulations.
• Develops the nursing plan of care for the purpose of delegating nursing directed patient care for patients receiving only home health aide services in the absence of a skilled service as required by Agency policy and State and/or Federal regulations.
• Performs supervisory visits for LPN's and HHA/CNA staff according to Agency policy, State, and Federal regulations.
• Attends, and actively participates in, case management meetings in person, by phone, or by other Agency approved method of communication.
• Ensures ongoing interdisciplinary assessment of the patient through case management and coordination with other members of the care team.
• Provides patient, caregiver, and family counseling; patient and caregiver education; prepares clinical notes; and communicates with physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of care.
• Assist with the creation and validity of the prior authorization (PA) request and ensure consistency between the PA, the comprehensive assessment, and the plan of care; including providing clerical and clinical support for all stages of the PA cycle.
• Coordinates all clinical documentation required for patient's file, to include but not limited to; signed plan of care, valid face to face, home health aide assignment sheets, case conferences.
• Assist Clinical auditor with open and closed chart reviews.
• Participates in the Agency's QAPI program as directed by the Clinical Manager and/or Administrator.
Additional position requirements can be requested. The salary for this role is competitive within the home health industry.
