POSITION: Clinical Coordinator
TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director
DEPARTMENT: Resident Care
STATUS: Non-Exempt/Hourly
SUMMARY
The Clinical Coordinator assists the Health and Wellbeing Director with administration and organization of the resident care department. Responsibilities include resident care transitions, clinical protocols, quality assurance, regulatory compliance and oversight of staff scheduling. The Clinical Coordinator ensures that policies and procedures established by Orchard are implemented and maintained consistently.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- High school diploma or GED
- Completion of a Professional Nursing Program and licensed to practice as a registered or licensed nurse is preferred or successful completion of a state approved nursing assistant training program and a state approved trained medication assistant program
- Current, active certification on the Georgia Nursing Assistant Registry and current certification in medication administration
- Must successfully clear a background check, TB skin test or x-ray, and health screening
- Current CPR certification
- Must have a valid driver's license
- Current certificate of training for successful completion of the five (5) hour initial fire safety training provided by the Rules and Regulations of the Georgia Safety Fire Commissioner
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Assists the Health and Wellbeing Director with administration and organization of the resident care department
- Coordinates and supports Health and Wellbeing Director with Personalized Service Plans and family care meetings
- Liaison between the community and residents, families, medical professionals, pharmacy, and vendors regarding resident care matters- Refers matters outside of the scope of position to the Health and Wellbeing Director
- Manages Family Help Line for resident families' direct access to the Resident Care Department
- Ensures proper follow through and documentation for residents with a change in care needs
- Maintains wellness files according to Orchard policy, federal and state guidelines
- Reports to Health and Wellbeing Director to identify, review, discuss potential changes in service / medication levels
- Manages and coordinates resident care appointments
- Informs the Health and Wellbeing Director of any changes in medication or service level that may result in a higher tier level or potential move-out
- Ensures weights and vital signs are obtained monthly for each resident prior to completion of monthly wellness visit
- Contacts resident's attending physician when necessary and or upon family request
- Maintains wellness supplies for the community daily needs and emergency kits
- Oversees the monitoring of each resident's medication and treatment profile to ensure each medication and treatment is administered as ordered and documented accurately
- Monitors the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications and expired medications
- Oversees nurse and care staffing scheduling are in accordance with the operational and budgetary guidelines
- Supports Health and Wellbeing Director with interviewing and onboarding new nurses, community supervisors, Certified Medication Assistants and Resident Assistants
- Supports in the facilitation of ongoing trainings and skills competencies of resident care staff
- Actively participates in risk management programs
- Balances team and individual responsibilities, exhibiting objectivity and openness to others' views
- Contributes to building a positive team spirit
- Identifies and resolves problems in a timely manner, gathering and analyzing information to develop alternative solutions both independently and with groups
- Manages challenging and / or emotional resident(s), family member(s), and employee situations, responding promptly to their request for service and assistance, while soliciting feedback to improve service insuring that commitments are met
- Focuses on solving conflict and not blaming
- Listens to others without interrupting and keeps emotions under control- Remains open to others' ideas and tries new things
- At times, provides assistance with assigned personal care tasks that meet resident physical and emotional personal needs consistent with the Orchard philosophy
- At times, administers prescribed medication as ordered by a physician to residents and maintain related medical records under the supervision of the RN

- Encourages staff to make suggestions on safety practices
- Participates in orientation of new employees
- Participates in volunteer orientations, training and education as requested
- Remains updated and communicates state regulation changes to all associates
- Ensures compliance with all state and federal regulations (i.e. OSHA)
- Participates in the risk management programs
- Demonstrates the ability to be innovative and creative and have patience, high energy and resourcefulness in problem solving
- Assists as requested by the Executive Director in a dining room during resident meal times to ensure residents have a positive experience
- Must always be accessible either by phone or email- Voice messages and emails are responded to within 24 hours
- Participates in the Manager on Duty (MOD) call rotation as requested by the Executive Director or Health and Wellbeing Director
- Ability to work evenings, weekends and flexible hours when necessary to meet the needs of our residents
- Follows Professional Dress Code policies as outlined in the employee handbook for community and department specific standards
- Adheres to Orchard's No Cell Phone policy as outlined in the employee handbook at all times
- Perform other related duties as assigned
