The Charles Lea Center is a national leader in providing supported living solutions so men and women with disabilities can live more independently. Additionally, we provide an array of other support services designed around supporting people to achieve their unique goals and interests. Depending upon the individual's needs and desires, Quali fied Professional with the help o f technology, provide the needed level of support and care, allowing the person to be more self-sufficient. Charles Lea Center staff is experienced at customizing support and programs to fit the indi vidual. In fact, it' s what we do best! We support and train our staff to understands the needs of men and women who are living alone for perhaps th e first time in their lives.

Acting as the first line of support to the amazing people and families served by The Charles Lea Center, you will work and communicate in an open, helpful, and engaging tone, to support others to take steps on their journey to independence.

Your Role: Our Qualified Professional works under the general supervision of the Director of Operations . Your skill set provides for the health , safety, accountability, supervision and training for people supported in the community. In this role you will assist people supported in a person-centered philosophy

Requirements :

Your Role: Y ou will be responsible for overseeing areas necessary to successfully support the program plans for individuals supported in Greenville and Wilmington, NC . The QP facilitates and fosters the Interdisciplinary Team Process which includes the development of CLC's service objectives and the individualized Service Plans (ISP) for people supported . Will collaborate with other team members and MCO CARE Coordinators to ensure effective and efficient program delivery to people supported . Follow through all documentation requirements for the Interdisciplinary Service Plans (IPS) ensuring all services are recorded, as required by the NC Dept. Of Health and Human Services and Managed Care Organization and any other assigned duties.
