Job Title : PT-Home Health

Location : La Crosse, KS 67548

Hourly Pay : $50 to $55 per hour

Shift : Days

Job Description:

We are currently seeking a dedicated and compassionate Physical Therapist (PT) to join our Home Health team in La Crosse, KS. As a PT in our Home Health program, you will play a crucial role in providing exceptional care to patients within the comfort of their own homes.

Responsibilities:

• Conduct comprehensive assessments of patients' physical abilities and limitations.

• Develop individualized treatment plans tailored to meet each patient's specific needs and goals.

• Provide hands-on therapy interventions to improve mobility, strength, balance, and overall functional abilities.

• Educate patients and their families on proper techniques for exercises and activities to promote optimal recovery and independence.

• Collaborate with other members of the healthcare team, including physicians, nurses, and occupational therapists, to ensure coordinated and effective care.

• Monitor and document patients' progress, adjusting treatment plans as needed to achieve desired outcomes.

• Maintain accurate and up-to-date patient records in accordance with regulatory requirements and organizational policies.

Qualifications:

• Valid license to practice as a Physical Therapist in the state of Kansas.
• Minimum of one year of experience working as a PT, preferably in a home health or similar setting.
• Strong interpersonal and communication skills, with the ability to build rapport with patients and their families.
• Demonstrated ability to work independently and manage time effectively.
• Commitment to providing high-quality, patient-centered care.
• Flexibility to travel to patients' homes within the La Crosse, KS area.
• CPR certification (preferred).
