Job description

UCFS is seeking a full-time Resident Attendant to work imbedded in our healthcare team to improve overall health status by providing care coordination and integration of services to patients with complex medical health needs and social services issues. This is a 32 hour per week, overnight, awake shift (11:00pm-7:00am) that includes every other weekend. This position will provide services through outreach and engagement to patients upon referrals using a strengths-based approach to assess patient needs. The successful candidate will also develop a patient-centered care plan that empowers and improves patients overall health.

Requirements:
- Home Health Aide or Certified Nursing Assistant (HHA/CNA) certification plus minimum1-3 years experience, or equivalent
- Medication Administration certification required
- Valid CT State Drivers License with good record
- Occasional night attendant coverage may be required to ensure licensing compliance
- Resident Attendants are required to remain on duty and available at all times including during their paid breaks
