Job Summary:
Lutheran Haven is seeking a Community Liaison to join our health concierge team. The Community Liaison's primary responsibilities are to assist with the development and monitoring of aging in place recommendations and developing and maintaining working relationships with Lutheran Haven residents, outside resources, and hospital case managers. The Community Liaison may be expected at times to visit Lutheran Haven residents in the hospital setting and skilled nursing facilities to assist with their return home. This is accomplished through the development and implementation of assisting with health care and wellbeing services to optimize quality services and access across the continuum of care.
Job Duties:
- Actively support Lutheran Haven’s Mission and Ministry.
- Demonstrate a willingness to be an effective team member by fostering positive relationships with teams and residents.
- Train in aspects of case management, including appropriate clinical and professional roles and ensuring a positive resolution to any areas of conflict, to ensure the efficient and effective delivery of multi-disciplinary care and create aging in place recommendations based on residents’ individual needs.
- Serve as a liaison with hospitalized residents and their families to help assure appropriate placement in post-acute circumstances.
- Communicate with Resident Case Management Coordinator and Director of Health Concierge to assure seamless continuity of care by acting as a patient advocate when presenting their needs and preferences for care, and actively promote the safeguarding policies and procedures to protect vulnerable people as necessary.
- Attend some community Independent Living (IL) functions to develop and maintain working relationships with Lutheran Haven residents. Set up 1 or 2 educational seminars a month which serve to advance community relations.
- Facilitate completion of long-term care insurance documentation/ VA application documents/ and Medicaid. Send to insurance companies completing VA or Medicaid applications. Assist with Social Security or other government agency calls, change of address forms if needed.
- Communicate effectively and work collaboratively with colleagues to ensure delivery of coordinated multi-disciplinary services to patients within the complexities of the Haven community environment.
- Assist with Assisted Living Facility (ALF) tours and admission process. Provide ALF and IL discharge planning assistance to off-campus facilities when needed.
- Other duties as assigned.
Minimum Qualifications:
- Minimum of two (2) years of experience in a similar role required.
- High school diploma or GED required.
- Associate’s or two-year college degree preferred.
