Requirements:

Bachelors degree in Social Work from an accredited institution

1 year experience

Have an active state of Kentucky license

Responsibilities:

Have experience with children with special needs

Screening and Evaluating a patient s condition and needs

Establishing treatment programs that cater to a patients needs and assist them in achieving specific goals

Evaluate and record progress for use in billing reporting and assessment

Provide Medicaid documentation direct services

Attend Individualized Education Program (IEP) meetings as necessary
