As a team member of our Senior Community Care (SCC) product, we work with a team to provide care to patients at home in a nursing home, assisted living for senior housing. This life-changing work adds a layer of support to improve access to care.

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. In this role you will have the ability to achieve work life balance. No on-call, no weekends, and no holidays required. This position requires travel through the state of OH,KY, IN.
