Job description
Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education and more.
Under the direct supervision of the Resource Specialist Supervisor, the Early Childhood Care Coordinator is an essential member of the health care team. This position plays a pivotal role in identifying whether pediatric patients are reaching developmental milestones. The Early Childhood Care Coordinator builds a strong relationship with the patient and family and answers questions about child development and wellbeing, so all babies and toddlers have a strong start in life. The Early Childhood Care Coordinator works closely with the pediatric care team to enhance and increase childhood developmental screenings utilizing an integrated and team-based approach.
Additionally, the position is responsible for promoting improved access to meet the needs of patients by aiding in navigating through TVHC’s healthcare system and providing brief care management as needed to link patients to developmental services. The Early Childhood Care Coordinator provides individualized assistance to ensure patients are informed of potential care options and links patients to all needed health and wellness services to promote optimal health. This position also provides timely and coordinated internal communications between medical and dental care, behavioral health, wellness programs staff and patients when needed.
This Early Childhood Care Coordinator utilizes the electronic health care record and population management tools to collect data, communicate with providers to support collaborative care, and improve health outcomes.

This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.
Compensation: $24.00 - $25.00 per hour, depending on experience.
Responsibilities:
Preventive and Responsive Care
- Conducts team-based well-child developmental screening support before, during, or following the primary care provider visit.
- Distribute educational materials and provide general consultation on parenting and child development themes for patients’ families, in consultation with the Child Development Specialist.
- Patient engagement focuses on monitoring community referrals management, parenting education regarding child development, and social-emotional functioning.
- Provide care coordination for TVHC patients in person, by phone and video conferencing.
- Fully knowledgeable of patient eligibility requirements for additional service and assist patients in navigating their care with both in-house and outside providers.
- Screens and addresses Social Determinants of health-related issues which create barriers to care, i.e., food and housing insecurity, transportation, and financial strain.
- Participates in Reflective Supervision with the Director of Care Coordination
- May co-facilitate parent/caregiver education and support groups with the Child Development Specialist.
- Promotes the importance of developmental screening in clinic with woman’s health and pediatrics.
- Making positive parenting and early learning guidance information available for pediatric patients and their families.
- Maintains open communication with all members of the primary care team.
- Works closely with pediatric primary providers and other patient care teams.
Referrals and Community Collaborations
- Provides referrals and tracks follow-up, as appropriate, to help families make successful connections to key resources within the community.
- Reaches out to community partners to share about their pediatric care manager role, and receive information to support referrals, warm handoffs, and ensure appropriate referral criteria is met for patients.
Team-Based Care
- Is an active member of the pediatric care team (Child Development Specialist, provider, MA, Resource Specialist and Panel Specialist) and attends regular team meetings.
- Works with the patient’s family and TVHC clinic staff to ensure seamless service integration of all eligible TVHC medical, dental, behavioral health and wellness programs.
- Accepts warm hand-offs between departments and coordinates care for patients to facilitate integration of care.
Caseload Tracking, Record Keeping, and Reporting
- Documents all patient clinical activity and care coordination in EHR.
- Provides brief early childhood care management services to a caseload made up of low-risk pediatric patients and their families to ensure connectivity to developmental services.
Requirements
- Bilingual in English and Spanish required.
- Associate degree required, in psychology, human development, social work, or counseling, or minimum of two years of relevant experience may be considered in lieu of degree.
- Bachelor’s in psychology, human development, social work, or counseling preferred.
- One year of care or case management experience working with families with young children preferred.
- Experience and knowledge about early childhood growth and development preferred.
- Parent-child relational health, infant and early childhood mental health, and family systems strongly recommended.
- One year working in a community clinic setting preferred.
- Ability to travel to designated locations as directed by supervisor required.

- Comfort with an open-door policy, potentially frequent interruptions, and warm hand offs
- Flexibility in communication: able to switch between being on the floor playing with young patients and communicating with program and medical professionals.
- Have great customer service skills; including the ability to active listen, communicate clearly, be empathic and have patience. Ability to self-regulate and de-escalate when working with patients.
