Salary Range: $50,872.50 - $58,172.50
Under the supervision of the Assistant Vice President (AVP) or covering AVP:
- Responsible for meeting with youth and family to develop individual success plans inclusive of the youth and
family’s goals, needs and wishes. In addition, the Social Worker can provide mental health counseling to
youth and family if that has been identified as a needed support. - Works with youth directly for 5 to 8 hours per week to discuss the youth’s goals, provide support in using the
extra learning, offer academic guidance (including on topics such as managing study time, sequential
learning, staying focused, and getting to know their learning style), help coach them in developing important
life skills (social skills, decision-making skills, health and safety, computer skills, etc.), and encourage
community/civic involvement. - Identifies various resources and supports that will help meet the individual needs of each youth and family.
- Provides mentoring success among high-risk populations, and includes one-on-one and group mentoring,
tutoring, life skills training, college preparation, and youth leadership training. - Responsible for the assigned caseload to build proficiency to the model and/or reach model fidelity, and
meeting the documentation requirement - Responsible for getting to know the participants’ teachers, peers, and family and make visits home to discuss
problems and find solutions. - Engages the youth by working with them to develop a plan, and providing them with rewards and
encouragement as they work toward their goal(s). - Undertakes advocacy and outreach with parents, teachers, the criminal justice system and other institutions
- Collaborates as team players and empower youth to do the same;
- Retains strong ties to the community;
- Organizes effectively to meet agency deadline and weekly contact requirements;
- Attends coaching and mentoring and group supervision with the supervisor at the main office, alternate
office, or in the community as needed. Come ready in supervision having planned and prep for supervision - Works closely with the social worker and supervisor to effectively support the youth and their families.
- Ensures adherence to all agency, City and State regulations and policies.
- Conduct intake, psychosocial, and on-going assessment, and developing treatment plan. Intakes and
psychosocial to be completed within program timelines and expectations. - Responsible for timeliness of documentation and data entry while maintaining confidential client records, and
meeting program deadlines. - Maintains clear, concise and accurate clinical approved documentation of treatment efforts that promote peer
and supervisory review and feedback, and that demonstrates compliance with evidenced based clinical model,
OMH, and meeting our agency’s expectation (i.e. Benefit Assignments, diagnosis, treatment plan, progress
notes, and on) - Assures documentation (data entry) for contacts, incidents, and reports are submitted timely (as indicated by
program). - Adopts a strength focused approach with families, stakeholder, and within the program/treatment team.
- Communicate advances and barriers experienced with the clients, team, and stakeholders, including
escalating any feedback received regarding own areas of improvement, to the supervisor - Plays an active role in development a Development plan that is tracked monthly to Quarterly.
- Participates in consultation with model developer consultant.
- Maintains communication with supervisor and escalate any incidents related to clients and/or staff and
complies with agency incident reporting policy. - Participates in all model program training, supervision and consultation activities
- Builds a relationship with the schools, FAP, and other stakeholder to ensure collaboration and proactive communication.
- Available 24/7 for the team and participates in weekend on-call rotation.
- Provides coverage support as needed to peers/team members at the main office, community, or other site.
- Able to respond to the needs of the team and program by shifting schedule to support
- Conducts presentation at stakeholder meeting on the program and services as needed
- Splits time between the main office and other offices, community based on meeting needs and client services needs
- Attends staff meeting and agency activities.
- Attends required training set by program, funding source, agency, and continued commitment to professional development
- Other duties as assigned.

Required Educational Level:
Bachelor’s level required in Social Work or relevant degree, including Education degree due to the nature of mentoring
Other Requirements:
- Bilingual (English/Spanish)
Preferred Experience:
- Possess at least two years of experience working in areas directly related to supporting youth, families or
communities. Mentors should come from, or be intimately familiar with the communities in which
participating youth/families reside. Mentors should have similar life experiences as the youth, and should
have a passion for working with adolescents. - Have experience in working with teens; undertake advocacy and outreach with parents, teachers, the
criminal justice system and other institutions; collaborate as team players; empower youth to do the same;
retain strong ties to the community; organize effectively; be computer literate; show patience with youth;
possess a sense of humor; and be able to think “out of the box.” - Knowledge of and experience providing treatment for domestic violence issues and substance abuse issues.
- Experience (internships through post-graduate) providing direct services to youth and their families in their
homes and communities - Experience working with youth and have knowledge of the juvenile justice system in New York City.
- Be proficient in Microsoft Word, Excel, PowerPoint, Internet, Email, and Data entry systems (e.g.
Connections, Electronic Health Record). - Excellent communication skills required.
- Experience in managing direct service providers who engage with the identified populations or similar
population in high-risk court-involved youth either in Family Court or Adult Court, Child-Welfare
programs. - An understanding and commitment to complying with the New York State Central Register of Abuse and
Neglect protocol (requiring staff to comply with promulgated requirements to report any suspected abuse,
maltreatment or neglect of any child). - Experience in working with or providing services to children and families, demonstrated ability to establish
trusting relationships - Experience with and willingness to work with culturally diverse population, computer literacy, able to
research current information on medical, social and legal issues. - Direct use of pragmatic (i.e. structural, strategic and functional) family therapies
- Behavioral therapy targeting school behavior and academic performance
- Implementation of interventions within or between systems in the youth’s natural ecology that affect or
influence the behavior of youth (i.e. family, peer, school, and neighborhood) - Creating and following a Treatment Plan
- Comfortable and adaptable to changes
- Take initiative and a team-player

- Accountability and open to ongoing evaluation, feedback and supervision of his or her work
- Experience with cross-agency collaboration
