Job description

The Case Manager for the Unaccompanied Children’s (UC) Shelter, under the supervision of the Lead Case Manager, acts as an advocate for the youth, supporting them in exercising their rights, choices, and responsibilities. The main role of the Case Manager is to ensure that each child is safe and cared for, and is reunited with a suitable parent, guardian, or sponsor in the most expeditious manner possible. This position requires the consistent exercise of discretion and judgment. The Case Manager is a grant position related solely to the Unaccompanied Children’s Shelter as funded by the Office of Refugee Resettlement and any successor funding.

ESSENTIAL FUNCTIONS:

• Ensure the safety of all children within each home;
• Coordinate and handle an estimated case load of 8-10 at any given time;
• Coordinate initial intake for each child;
• Schedule and confirm each child has an initial medical evaluation within 48 hours of arrival;
• Coordinate and assist the clinician to assure the admission assessment and family tree are completed within 3 days of arrival;
• Coordinate and assist clinician to assure the psychosocial summary and individualized Service Plan (ISP) are completed within 7 days of arrival;
• Manage reunification process for each child;
• Keep information updated in UC portal;
• Coordinate the discharge and release of child to their sponsor;
• Respond to client emergencies within or outside work hours;
• Other duties as assigned.

MINIMUM REQUIREMENTS:

Education/Experience:

• Bachelor’s degree in relevant behavioral/social sciences field;
• 2-4 years progressive experience in case management.

Skills /Knowledge:

• Bilingual English/Spanish language competency required;
• Strong communication and organizational skills;
• Strong skills in Microsoft Office and web-based databases;
• Must possess a valid driver’s license.

Physical Demands:

• Required to frequently sit, stand, bend, stoop and walk, sometimes for extended periods; able to walk up and down stairs;
• Required to lift/move objects and/or youth weighing in excess of 40 pounds;
• Must have visual, hearing, and learning capabilities sufficient to perform the essential functions defined above.

HIPAA PRIVACY TRAINING AND SANCTIONS:

All employees who have access to health information whose confidentiality is protected by the HIPAA Privacy Rule are required by the Rule to receive training so that they have an understanding and knowledge of the Privacy Rule that corresponds to their job responsibilities and of the group health plan’s policies and procedures that impact on their job duties. Site specific training on requirements of the HIPAA Privacy Rule and the plan’s health information policies and procedures under the direction of the plan privacy officer may be required for this position based upon site specific requirements. Employees who violate the requirements of the HIPAA Privacy Rule will be subject to discipline, up to and including termination.
