JOB TITLE:                   SOAR Case Manager

DEPARTMENT:             Adult Housing & Services

FLSA:                           Non-Exempt

LOCATION:                Yonkers ,NY

REPORTS TO:            Program Coordinator/Director

SALARY:                    $40,000 TO $45,000 PER YEAR

SUMMARY :

The SOAR Case Manager is responsible for assisting Westhab’s consumers apply for social security, state disability benefits and other entitlements. Works closely with other staff to help consumers access a continuum of care that addresses their health care needs and other critical resources such as long-term care, disability benefits, employment opportunities, job training, and permanent housing. Provides case management to help client access available resources, transportation coordination, information and referral, crisis intervention, and advocacy. Acts as liaison to other community service provider agencies.

DUTIES & RESPONSIBILITIES:

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

Must have SOAR certification or complete within two weeks of hiring.  Associate’s or Bachelor’s degree in social work or related field preferred, with at least two years of experience in direct client services.    Strong computer, presentation, written and verbal communication skills required.  Must maintain ongoing knowledge of community dynamics and possess an in-depth familiarity with areas in Yonkers.  Must have the ability to develop strong community partnerships with local service providers, businesses and law enforcement.  Some evenings, weekends and flexible scheduling when needed to meet the needs of the program.  Must have own transportation and valid drivers license.
