Job description
Facility Job Description: 1. Clinical Care a. Provides case management activities for identified patient population, including assessing and managing patients, implementing clinical practice guidelines, evaluating outcomes and modifying plans in consultation with other members of the health care team. Ensures continuity of care by providing services that encompass the entire spectrum of care, from inpatient care through discharge to outpatient care. b. Provides and/or directs the delivery of nursing care for the assigned population. Consults with other specialists as required. c. Monitors medical condition of patients and reports changes to the appropriate personnel. d. Provides telephonic triage and advice services for assigned population. e. Participates as a member of the interdisciplinary team in the development of the patient plan of care. Assumes responsibility for coordinating the management of the plan of care. Incorporates patient and family priorities into the plan of care. f. Consults with and advises inpatient nursing unit staff regarding pertinent patient management issues. g. Interacts with other members of the health care team and facilitates the recognition and resolution of patient problems for assigned caseload. h. Prepares patients for scheduled procedures. i. Evaluates outcomes of patient care, consults with other specialists as required and adjusts patients plan of care as necessary to meet patient care goal. j. Facilitates in the discharge planning process for assigned population through consultation with members of the healthcare team. Ensures patient discharges occur in a timely manner. k. Provides support for patients/families. Utilizes crisis management techniques as appropriate. l. Performs accurate documentation in medical record.
