Qualifications of an ideal  Case Management Director :

• LPN preferred
• Bachelor’s degree in human services, social work, psychology or a related subject is preferred
• Experience in the field of human services or social work providing services to an adult population as required by state regulation if applicable.
• Two to five years’ experience in human services or social work providing services to an adult population is preferred

Case Management  Director Job Summary:

Case Management Director is responsible for the operations of the Assisted Living setting in accordance with regulations and standards.

• Participates in planning for pre-admission and discharge.
• Assists residents and families with the move in process and orientation to community by coordinating with the marketing and nursing teams
• Participating as a member of the interdisciplinary team in resident care planning
• Plan, coordinate, and administer the new Resident orientation program. Monitor and evaluate necessary changes and/or improvements.
• Documents case management notes in residents’ files as necessary.
• Invite and encourage resident attends their care planning session and ensures their preferences are honored
