Job description

Our staff is searching for a skilled clinical coordinator to join our team and help grow the Fetal Diagnostic and Treatment Center at Atlantic Health. The Atlantic Fetal Diagnostic Testing Center (AFDTC) is a multidisciplinary program that serves as a resource for patients who receive a fetal diagnosis in utero. A partnership of fetal and pediatric subspecialists, the AFDTC provides comprehensive care to women and fetuses with complex medical needs that includes additional prenatal ultrasounds, fetal echocardiograms, and subspecialty consultations to optimize post-natal outcomes. In this position, it will be your responsibility to work one-on-one with patients to tailor individualized multidisciplinary care teams. The nurse care navigator will optimize the patient experience through clinical coordinating and collaboration. Ensuring that expecting parents meet the appropriate subspecialists in the appropriate sequence during their antenatal course and maintain communication among the care team. You will help patients navigate complex and stressful diagnoses through providing exceptional care and guidance for the duration of their pregnancy, and in the immediate post-natal period.

To do well in this role as a member of the AFDTC team, you will need to have an excellent understanding of medical terminology. Additionally, you should have strong organizational, communication, empathy, and leadership skills. 

Nurse Care Navigator Responsibilities:

• Work with medical staff to create patient care plans
• Work one-on-one with patients and newborn to ensure their needs are met
• Develop and implement policies and protocols that will ensure the success of our program
• Serve as point of contact between providers and patients
• Be a familiar, reliable, and accessible resource for patients as they navigate a stressful time
• Obtain patient outcomes after delivery 
• Ensure that all HIPAA regulations are followed

• Bachelor’s/Master’s degree in nursing
• Previous experience in pediatrics, maternal fetal medicine, or obstetrics preferred. 
• Prior healthcare administration experience would also contribute to success in this role.
• Working knowledge of key medical terminology and willingness to learn about the clinical aspects of perinatal care.
• Excellent communication, empathy, and leadership skills
• Skill at implementing new protocols and procedures to optimize efficiency
