Job description
Hospice Case Manager – Covington GA
We have recently engaged with a very progressive, well run and financially stable Hospice organization in the Covington area of Georgia. They are a family-owned organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.
Our client is offering the following benefits:
- Annual Salary: $80,000-$90,000
- Health Insurance-Employer Pays % COBRA
- Dental And Vision Insurance – Employer Pays % COBRA
- Mileage Reimbursement-$0.48 Per Mile
- PTO-14 Days
- Paid Holidays- 7 Federal
Responsibilities:
- The RN Case Manager Will Maintain a Case Load of About 12-14 Patients
- Patients Per Day-4 on Average
- The RN Case Manager Will be Responsible for Back-up Call
Requirements:
- Registered Nurse in the State of Georgia
- Must have MatrixCare Experience
- 2-3 year of Hospice or Palliative Care Experience-Preferred
- 2 years of in-Home Patient Care Experience-Required
- Familiarity with EMR System MatrixCare-Preferred
