Job description
The REM Team Manager supervises a team of Clinical Care Coordinators providing care management services to individuals in the Rare and Expensive Case Management (REM) Program. The REM Team Manager supports the mission of The Coordinating Center by providing person and family-centered care management within the parameters of the REM program. Team Managers are responsible for monitoring compliance with specific contract requirements, integrating technology to maintain database integrity and providing technical and analytical support in contract planning, data extraction, report writing and maintenance of all documentation. This position works interdependently with The Center’s Care Management leadership team to maximize quality outcomes for individuals receiving REM services. The REM Program Director directly supports the REM Team Manager position providing informal and formal performance feedback.

Essential Responsibilities
Assumes day-to-day coordination of delegated REM program functions and provides direct supervision to team members.
Serves as a resource to staff for problem solving regarding client issues, contract requirements, policies and procedures, DME/DMS, eligibility; advises, supports and takes appropriate action.
Coordinate and manage external and internal communications and resources to maintain quality service delivery for teams.
Facilitate effective teamwork and maintain an efficient and respectful work environment.
Act as first point of contact with Coordinators and Maryland Department of Health regarding problems concerning care management issues, contract requirements, policies and procedures and other issues. Fields questions to Program Manager and Program Director as appropriate.
Monitors Coordinator’s work and documentation to ensure that clients are receiving quality care management.
Ensures timely completion of all performance evaluations and required documentation to reflect and track coworker performance. Troubleshoots staffing issues with Program Manager/Director and takes appropriate action.
Assure that clients assigned to team have an assigned Clinical Care Coordinator and/or have a point of contact when the CCC is not available, ensuring the team is appropriately covered.
Maintains a smaller client caseload and delivers care coordination services per The Center’s protocols adhering to the contract requirements.
Attends meetings when appropriate, in Program Manager/Director’s absence.
Communicate with other Team Managers to provide consistency across teams and programs.
Interview, orient, educate, and mentor care management staff on care management concepts, service delivery and REM contract specific processes, tools, and procedures.
Continually assesses the resources, learning opportunities, and training needed by staff. Communicates those needs to the Director for discussion and implementation of the course of action.
Manage multiple priorities and tasks with flexibility to enhance Care Management.
Maintain reports as contracts require ensuring accuracy and following up with Coordinators as needed. Assist with periodic contract audits.
Collaborate with Human Resources (HR), Program Manager and Program Director in order to promote compliance with HR policies.
Leads by example and implements The Center’s mission/values within the department.
Qualifications
Licensed RN or Social Worker
CCM or equivalent
Bachelor’s degree from four-year college or university
Minimum five years Care Management experience working with individuals who are medically complex
Two years of supervisory experience required
Excellent communication skills-writing and speaking.
Computer skills including Word, Excel, CARMA and PowerPoint
Creative problem solving and flexibility
Training or mentoring experience
Ability to manage complicated work processes
Ability to communicate effectively with other professionals, as well as clients and their families
