Sign-on Bonus $3,000
Schedule: Monday - Friday 8 am - 4:30 pm
with weekend hours every 2 months
The Hampton - Newport News Community Services Board is seeking a Service Coordinator to join our team in the Assertive Community Treatment (ACT) program. As a Service Coordinator, you will play a critical role in ensuring the continuity of care for our clients, who may have co-occurring intellectual developmental disabilities and substance abuse issues. You will provide essential support to our clients by addressing their physical, financial, social, and personal needs. This role includes various responsibilities such as outreach, assessments, treatment planning, counseling, and crisis intervention, with the aim of improving the well-being of our clients and helping them lead more fulfilling lives.
Key Responsibilities:
Requirements:
Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling, with one (1) year of experience in case management or service coordination services. Master's degree preferred. Must be registered with the Board of Counseling to practice as a QMHP-A or register with the Board of Counseling to practice as a QMHP-A trainee within the first 30 days of employment. Valid Driver's Licenses
Current HNNCSB are not eligible for the Sign-on Bonus
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
