UPMC Home Healthcare is hiring caring, skilled, and compassionate registered nurses to join our home health nursing team! You’ll work in a home setting with a diverse patient population throughout the Blair and surrounding area . Working outside of a medical facility, you’ll have an opportunity to get to know patients as you guide their care in the comfort of their homes.
8:00pm-8:00am Nights (12 hour shifts).
As an RN with UPMC Home Healthcare, you’ll collaborate with our team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to provide personalized patient care in a home setting.

Responsibilities:
- Demonstrates a passion for developing others, enthusiastically and expertly precepts students, graduate nurses, and new hires. Provides creative and innovative approaches to direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care. Assumes responsibility for complex patient assignments; views problems as challenges and helps peers to do likewise. Initiates, coordinates, and evaluates patient and family centered health teaching.Promotes change that enhances the quality of patient care and the environment.Focuses on customer service and continually strive to perform job duties in a manner that results in optimal patient satisfaction. Assign other nursing personnel, including LPNs and Aides to assist in the appropriate delivery of services as ordered on the plan of care to the patient. Supervises and teaches other nursing personnel including LPNs and Home Health Aides. Influences behavior to create positive change and utilizes alternative leadership styles as necessary. Identify, investigate, and recommend solutions to organization issues. Identify any deficits or exceptional performance issues of other nursing personnel, LPNs and aides, and recommend solutions.
- Function as an expert within clinical specialty areas of certification and incorporate evidence-based nursing into projects and practice environment. Attend compliance training and adhere to organization standards of conduct, policies, and procedures.Initiates and coordinates case management activities within a multi-disciplinary health care team to ensure appropriate delivery of services to patient. Interdisciplinary relationships are fostered by negotiation and consensus building for attainment of goals for all. Attends and participates in patient care conferences, team meetings and other related meetings. Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC Home Care Agency towards patients, visitors, staff, peers, physicians, and other departments within the organization. Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.

Establishes the plan of care in collaboration with the physician, patient, and UM Professional in accordance with the physician orders and patient needs. Advises and consults with family and other agency personnel, as appropriate. Completes documentation including visit assessment and verbal orders accurately and promptly on the laptop device or in written documentation while in the patient’s home and within the agency’s and regulatory standards. Data syncs (transmits) information same day. Anticipates/responds and exercises independent judgment in matters concerning emergent and non-emergent patient care needs and communicate with the physician. Participates in on-call coverage rotation for the agency.
