PAY RANGE
$79,000 - $85,000 Annual Salary - Pay based on applicable experience and qualifications
POSITION OVERVIEW
Under the leadership of Sr. Director Supportive Housing, the CAL AIM Program Manager provide primary oversight of all CAL AIM services. This is a dedicated Cal AIM role and does not directly supervise regional staff, although it entails significant interaction with / support of Cal AIM site staff both regionally and within Health & Supportive Housing.
This is a hybrid position, based on current MidPen policy of working Tuesdays and Thursdays in the Foster City Office. Mon, Wed and Fri, option to work from home or the office. When working from home, requires a reliable internet connection.
ESSENTIAL DUTIES
CalAIM Operations Management
Establish consistent routine (daily or weekly)
- Stay up to date with emails, teams and task follow up
- Check incoming faxes for approvals, authorizations, and referral statuses
- Coordinate with site staff on correction/actions needed for file review/authorizations
- Maintain MidPen Cal AIM tracker
- Review CCAH portal and HPSM portal
- Check status on new ECM referrals and new authorizations
- Manage reauthorization schedules and paperwork readiness
- Monitor Medi-Cal eligibility
- Follow up on cases and support needed for team
- Follow up on unit vacancies and collaborate with 3rd parties and Property Management
- Oversee Salesforce QA reporting
- Review consent, service plans and documentation (QA resident monthly wellness checks and service provided)
On a Monthly Basis:
- Check SFTP files for receivables
- Review encounter file submission for Docustream with Data Analyst
- Send the reauthorization list to managers for the following month
- Follow up with HPSM CCAH as needed on program issues
- Follow up with partner on cases
- Follow up on claims files
- Review and reconcile payments and capitation reports
- Review potential additions of residents in need of CAL AIM support
- Review number of new referrals made to the CAL AIM DOH units
Cal AIM Site Staff Support
- Facilitate biweekly team meetings
- Manage CalAIM onboarding for new staff
- Update documentation protocols
- Documentation and service plan reviews
- 1:1 support for high need cases, case conferences, problem solving, resource connections and referrals; escalate to Sr. Director, Supportive Housing, as needed
- Maintain CalAIM Teams channel, fax and SharePoint sites
- Support authorization and reauthorization processes
- Vacancy management and advocacy with referring agencies
CalAIM Partnership Collaboration
- Attend regular monthly meetings and collaborate with CCAH and HPSM
- Encounter reports, claims and billing, and disenrollment's
- New referrals and reauthorizations
- Clearinghouse collaborations
- CCAH Chart review submissions.
- Review Service plans pertaining to high-risk, high need cases and advise on-site staff for changes or updates as needed
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The requirements listed below are representative of the education, knowledge, skill, and/or ability required.
Education and Experience
- M.S.W. or M.A. degree in psychology, counseling, or related field and 2 years of full-time experience serving the needs of senior populations and special needs (mental illness, substance use, co-occurring disorders) or B.A. or B.S. degree in the same fields with 3 years full-time experience serving these populations or an equivalent combination of education and experience
- Minimum of two (2) years of experience in program development, management, and evaluation required
- Minimum of two (2) years of experience in managing or directing people and projects across multiple partners and teams
- Experience working with and serving homeless populations and/or providing social services to diverse populations
- Experience working with high needs or high acuity populations that have experienced trauma and/or homelessness including Medi-Cal programs
- Experience using and administering multiple assessment tools and interpreting information/data derived from assessments
- Experience supporting program and service implementation and coordination
- Experience collaborating with local service providers
Knowledge, Skills, and Abilities
- Extensive knowledge of Medi-Cal state and federal laws, rules, and regulations
- Familiarity with common mental health diagnoses and disorders and interventions including harm reduction, motivational coaching, and trauma informed practices
- Exceptional communication and interpersonal skills
- Ability to work collaboratively with teams across payor sites (HPSM and CCAH) and adapt appropriately to different staff roles (Service Coordinator and Case Managers)
- Resourceful and problem solver. Offers practical, easy-to-follow guidelines to staff, knows when to escalate situations to supervisor
- Excellent relationship-building, active listening, negotiation, and conflict-resolution skills with a demonstrated ability to maintain balanced professional boundaries
- Excellent attention to detail and organizational skills and demonstrated ability to work independently and consultatively as part of a team
- Effectively use Outlook, OneDrive, Teams, Zoom, Salesforce, Microsoft Office Suite, and other technology tools to support interactions with peers and supervisor
- Manages accuracy of data across various payor portals and MidPen systems such as trackers, salesforce, claims files etc.
- Diligently follows up with payors on receivables and maintains prompt communication.
- Promotes teamwork
- Strong time management and project management skills
- Must possess a valid California Driver’s License, proof of current auto insurance, and reliable transportation
- Ability to travel between assigned properties. Requires at least 40% of time in the field
