**Locums Plastic Surgery opening in Fort Wayne, IN**

**Locum to Permanent**
The position will start out as a locums assignment and if it's a fit, they would like for the provider to transition to permanent.

* Dates: ASAP - ongoing
* Rates: All inclusive $2800/day
* Includes Burn & Wound Care
* EMR: Cerner
* Allowed holidays: New Year's Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Day, & Christmas Day

Inpatient
* No call; Scheduled Clinical Hours Only
* Locum to perm
* No admissions
* Plastics/Burn and Wound Care
* Cerner EMR

**Requirements**
* Active IN license (no IMLC)
* Board certification
* Clean history
