Job Description
The Care Coordinator is part of a multidisciplinary clinical care team and responsible for collaboration with clinical team members, patients, and families to facilitate unmet health care needs within the context of the Patient Centered Medical Home (PCMH) model at NATIVE HEALTH. The Care Coordinator answers all calls from patients with clinical questions, providing excellent customer service and resolving patient inquiries in a timely manner.
KEY ACCOUNTABILITIES :
- Answers Health Services patient inquiries from multiple entry points and resolves the patient’s needs on a timely basis.
- Provides Care Coordination.
- Promotes Preventive Care and Chronic Disease Follow Up.
- Promotes the Patient Centered Medical Home
- Supports the organization as needed.
Qualifications
QUALIFICATIONS
/EXPERIENCE/REQUIREMENTS:
- Associate degree in a health-related field is required.
- 2 years of experience in the medical field is required.
- Ability to read, comprehend and write instructions, correspondence, and memos.
- Fluency in both English/Spanish preferred.
- Ability to gather data in an organized fashion from varied sources.
- Possess a valid and unrestricted Arizona (AZ) driver's license (within 30 days of hire if applicant is out of state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid AZ driver’s license during employment. Must be 25 years of age and insurable by NATIVE HEALTH liability auto policy.
- Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
- Must pass a substance abuse testing upon employment and submit to a random testing during employment.
- Must have an updated Immunization (IZ) Record.
- Must submit a recent report of Tuberculosis (TB) skin test upon employment and provide a TB skin annually.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of case management/care coordination techniques in a primary care setting.
- Knowledge of human behavior during stressful situations to exercise emotional control, tact, patience, and reliability in carrying out patient care.
- Knowledge of existing resources for services and ability to research and find the necessary resources.
- Knowledge of program planning concepts and practices.
- Knowledge in health education theories, concepts, principles, and practices specifically in diabetes, cardiovascular, cancer, stroke, and pulmonary disease.
- Knowledge of chronic care model.
- Knowledge of the cultural, social, and economic backgrounds of the various ethnic or race to provide culturally appropriate care in the clinic setting.
- Demonstrated ability to work effectively with all levels of staff including clinical providers, patients, and the public.
- Ability to collect, organize, record, and communicate, in a meaningful way, data relevant to clinical case management.
- Ability to provide individual and family counseling, guidance, and health instruction to help patients to understand how to prevent disease and maintain good physical and mental health.
- Strong organizations skills with ability to work efficiently to coordinate care for multiple patients at a time.
- Ability to work independently and take initiative in addressing concerns and issues.
