Description DEFINITION:
Under Provider direction, plans, organizes, coordinates and directs assigned professional occupational therapy services as a member of the interdisciplinary team providing support and information within the area of Occupational Therapy for Grand Lake Mental Health Center, Inc. The Department Head for this position is the Chief Administrative Officer. This position is immediately supervised by the Integrated Health Services Director
JOB DUTIES AND RESPONSIBILITIES:
Responsible and immediately supervised by the Integrated Health Services Director.
This position is responsible and held accountable for the following duties:
• To work under the direction of the Integrated Health Services Director.
• Review available client information and evaluate information relevant to the assessment and treatment process with prescribing physician.
• Conduct and document a thorough functional assessment of the client, utilizing standardized procedure.
• Develop and implement an individualized treatment plan based on client needs and containing achievable functional goals.
• Daily patient occupational therapy treatment.
• Comply with evaluation, treatment and documentation guidelines including concurrent documentation when applicable.
• Review and revise treatment plans, as needed, throughout the treatment process.
• Orient clients to program services and treatment procedures appropriate for their needs. Inform clients of any potential risk during any procedure.
• Work with nursing staff, clients and other appropriate parties to maximize client's use of functional abilities.
• Establish and instruct client and family/other caregiver in exercises and therapeutic procedures to be continued in the place of resident until next clinic appointment.
• Assist in the referral process when a client requires additional services outside of GLMHC's scope of care.
• Attend and participate or provide information for rehabilitation team staffing regarding client progress, problems or needs.
• Ensure a safe and clean facility including physical equipment along with maintenance and supply inventory upkeep.
• To maintain current clinical competency by continued education in nutrition and dietetics.
• Maintains appropriate licensure requirements, if applicable.
• Understands and promotes our Certified Community Behavioral Health Clinic (CCBHC) concept by providing or supporting community-based mental and substance use disorder services; advancing integration of behavioral health with physical health care; assimilating and utilizing evidence-based practices on a more consistent basis; and promoting improved access to high quality care.
• Regular and predictable attendance is an essential job requirement.
• Employee must be willing and able to perform all job-related travel normally associated with this position.
• Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.
• Participates in staff meetings.
• Promotes the programs and services of GLMHC through formal and informal interactions with the community.
• Adhere to all Agency policies and procedures.
• Must meet "satisfactory" expectations at any scheduled job performance evaluation.
• Quality of work will be evaluated through training, skill, or special purpose. Must be competent and adequate.
Must perform the specific job duties as listed above to meet position expectations.
In addition, must possess skill in working cooperatively and effectively with clients, staff, management and other professional groups; in exercising mature judgement in dealing with people; in presenting ideas clearly and accurately; in reading and comprehending the English language; and in communicating effectively, both orally and in writing.
Qualifications EDUCATION AND EXPERIENCE:
Graduated from an accredited college with a Bachelor's, Masters or Doctoral Degree in Occupational Therapy. Possession of valid licensure, certification or registration as an Occupational Therapist. Must be credentialed/credential eligible by the American Occupational Therapy Association. Preferred experience would be 1 year or greater in an occupational therapy setting.
KNOWLEDGE AND SKILLS:
Strong interpersonal skills and ability to communicate in a friendly and professional manner with both clients and GLMHC staff members
Ability to assess a client's occupational needs and make appropriate referrals for medical treatment.
Strong organizational skills and the ability to accurately document information using software such as Excel, Word and other Windows based programs, as well as our billing systems.
Have a general knowledge of the community mental health system and awareness of local referral sources.
An expectation of core competencies in relation to individuals with co-occurring disorders.
