Division Description
The Children, Youth, Family and Transitional Age Youth services Division (CYFTAY) offers an array of services to low-income and underserved clients with multiple challenges and abilities: Infants, toddlers, and preschool children, including those with special needs. Youth: including transitional age youth, pregnant| parenting teens and youth in the foster care system. Families: including low income, immigrant, monolingual, and homeless populations. The CYFTAY division offers multidisciplinary programs including Early Childhood Education, Early Intervention| Inclusion, Family Support, Mental Health Services, Health, Nutrition, Wellness and Parenting Education Programming.

Program Description

Feltons Early Care and Education programs provide comprehensive family support including case management program for parents/families enrolled in our ECE programs. Family Support case manager/s help ensure that families enrolled in our ECE programs have access to all available health, education, and social services for which they are eligible. They work with families to do an initial intake of family needs and co-develop an individual service plan for the family. Focusing on access or enrollment into healthcare programs for their children/family, enrollment in public benefits and basic need resources available on site and within our network of community partners. Families are also provided referrals to access postsecondary education, vocational opportunities and using the five protective factors with the families we serve. If the parent is pregnant, we educate and promote, reduction of low birth-weight babies, and reduction of risk behaviors, also provides a myriad of support services, including child development education, parenting education, childcare support services, nutrition, health education, job readiness and mental health program referrals, relationship violence prevention, and academic counseling. These support services are provided as part of Feltons FRC services and through longstanding partnerships with other youth-serving agencies.

The FRCs at Felton Institute consist of two comprehensive Family Resource Centers (FRC) with a multitude of wraparound services to serve the young parent population citywide and families enrolled in the early care and education programs. Our FRCs are inclusive of all cultural backgrounds and family composition. Key programming includes: Intensive care management, support groups that encourage parent leadership and healthy family development, Workshops and classes focused on child and youth development, health and wellness, Family Engagement, Community events, Curriculum based parenting workshops (STEP, PIPE, Abriendo Puertasa Latino-focused curriculum centered on family strengthening), and parent leadership/ advocacy, mental health support and counseling, family economic success, fatherhood programming, GED preparation, post-secondary education, support applying to programs such as Cal-fresh, Food Stamps, and Below Market Rate housing.

Job Summary

The Case Manager is responsible for working with families enrolled in our Early Care and Education (ECE) Programs. The Case Manager provides family advocacy, resource and referral and ensures that his/her assigned clients have a coordinated range of client-centered services linking them with health care, educational and social services. The Case Manager is responsible for completing initial and biannual assessments with clients to include completion of a care plan in addition to seeing clients periodically to address their current needs and checking on progress. The Case Manager is responsible for entering all client data, units of service, and case notes into the database in a timely manner. As assigned, the case manager will participate in family support services network meetings and support family support program activities. The Case Manager reports to the Family Support Supervisor. This position non-exempt, full-time.

Evaluates needs of clients and develops treatment plan in coordination with others for individuals with mental, emotional, or substance abuse problems. May also assist with family, parenting, and marital problems; suicide; stress management; problems with self-esteem and assisting clients in applying for and obtaining other social services while providing a safe and supportive environment for agencys clientele and adhering to laws regarding confidentiality and reporting requirements. Activities may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education. Maintain accurate detailed observation and progress records for electronic billing data entry.
