Job description
Overview The Suicide Prevention Case Manager provides brief, telephone-based outreaches to patients identified at risk of suicide after discharge from Wellstar Health System. The Suicide Prevention Case Manager meets with identified patients prior to discharge to orient patients to the Suicide Prevention Program in Wellstar Health System.
The Suicide Prevention Case Manager collaborates with team members primarily at Wellstar Behavioral Health Unit, the Wellstar Medical Group, and WellStar Hospital Floors & Emergency Departments, to support patients through their transition from a Wellstar visit to establishing care with outpatient providers.
The Suicide Prevention Case Manager conducts in-service suicide prevention training to Wellstar team members in the forms of continuing education and computer-based learning. The Suicide Prevention Case Manager may develop these training courses independently or in collaboration with other team members.
The Suicide Prevention Case Manager provides supportive outreach phone calls: conducting risk assessments and safety plan interventions and helping patients navigate challenges in accessing outpatient behavioral health care. The Suicide Prevention Case Manager also provides suicide risk telephonic screenings to patients who are currently at risk for suicide in the ambulatory setting and provides next level of care recommendations.
The Suicide Prevention Case Manager may serve as a liaison between the patient, family, Wellstar Health System, and outside agencies to increase safety and prevent suicide after discharge. The Suicide Prevention Case Manager provides resources, referrals and psychoeducation to patients and their support systems as needed.
Responsibilities Core Responsibilites and Essential Functions
- CASE MANAGEMENT
- Develops and maintains a network of community resources in order to facilitate referrals and linkages to community services for patients identified as at risk for suicide or non-suicidal self-directed violence post-discharge from the Emergency Department and inpatient units.
- Consults and collaborates with treatment team members working with patients identified as at risk for suicide or non-suicidal self-directed violence to determine patient’s resource needs.
- Collaborates with behavioral health assessors, PET Assessors and behavioral health liaisons to ensure completion of the Stress and Safety Management Plan to assist with managing patient safety following transition from Emergency Departments or inpatient settings.
- Reviews, assess and tracks the completion rates of the safety plan and suicide care plan.
- Documents clinical findings, resource needs and linkages in a timely manner.
- Communicates progress to all appropriate team members.
- Develops and maintains a registry of high-risk patients and facilitates care with other team members as needed.
- Provide patient/family education according to learning needs.
- Ensures each patient identified as high-risk for suicide has a completed Suicide Care Plan.
- Provide telephonic case management to identified patients within 24-72 hours of discharge from the inpatient unit or Emergency Department.
- Document calls/referrals into computer tracking system according to procedure.
- COMMUNICATION

- SELF-DEVELOPMENT
- Seeks opportunities to foster skills and professional development through in-services, rounds, web cats, computer-based learning modules, meetings and peer to peer knowledge sharing.
- Maintains current BLS, professional licensure and completes all required training to maintain job specific skills.
- Leader of Learning
- Development and implementation of the best practice training to behavioral health and non-behavioral health team members regarding the care of patients who are at risk for suicide and non-suicidal self-directed violence. Training activities might include:
- Provide QPR (Question Persuade Refer)
- Provide MHFA (Mental Health First Aid Training)
- Provide AMSR (Assessment and Management Suicide Risk)
- Provide other trainings/Computer Base Learning (CBL) as needed
- Development of learning outcomes for program participants, data entry and analysis, utilization of online platforms in delivery of evidence-based trainings.
- Participates in the change management process through implementation of educational initiatives with a focus on sustainability of practice and behavior change.
- Supports the systems competency model annually and at onboarding to ensure that team members are supported in the process.

Qualifications Required Minimum Education
- Master's Degree in Mental Health Counseling or related field from a CACREP accredited program
- Master's Degree in Marriage and Family Therapy from a COAMFTE accrediated program
- Master's Degree in Social Work from an Accredited School of Social Work
Required Minimum Experience
- Minimum 3 years experience working with adult/child populations Required and candidates have experience working with behavioral health patients. (internships and volunteer experiences may apply)
Required Minimum Skills
- Demonstrated counseling skills, knowledge of community resources, federal and local guidelines, funding resources;
- understanding of adult and geriatric development and age appropriate care
- understanding of the effects of illness and disability and the impact on both patient and family.
- Demonstrates skills or experience in case management.
- Strong computer skills required.
- Good communication skills both verbal and written.
- Excellent time management/organizational skills; working knowledge of the precertification process; Working knowledge of medical terminology and basic computer proficiency required
