Integrated Health 360 is a primary care practice serving patients across the states of Delaware and Maryland. We are aligned to ensure our communities can access patient-centered, medically oriented full-service medical care delivered in convenient locations with friendly, knowledgeable providers and staff. We are looking to expand our Primary Care Division.

 
Position Overview: 

• As a Nurse Practitioner you will support our Primary Care Division. In our fast-paced environment, you will evaluate and treat patients for a variety of conditions including HTN, Diabetes, High Cholesterol, COPD, etc. while delivering a great experience and top-notch patient care that our patients have come to expect from Integrated Health 360.
• You will have the support of medical assistants, clerical staff, and a billing team, as well as other tools to make documentation and treating patients easy. 

Hours: Monday- Friday 8-4pm, No call or weekends.

Candidate Requirements: 

• Licensed in Delaware and willing to obtain Maryland License 
• Experience in Primary Care preferred.
• Strong assessment and communication skills.
• Knowledge in medication management.
